INTRODUCTION
1.1 Background of the Study
Clinical laboratory services play a critical role in modern healthcare delivery, as approximately 70–80% of medical diagnoses and treatment decisions depend on laboratory test results. Among these services, the haematology department is one of the most essential, as it deals with the analysis of blood and blood-forming tissues used in diagnosing conditions such as anaemia, infections, leukaemia, bleeding disorders, and other haematological abnormalities. Because of the direct impact of haematology results on patient care, ensuring accuracy, reliability, and timeliness of laboratory findings is fundamental to effective healthcare delivery (World Health Organization, 2022).
To achieve reliable results, laboratories must implement robust Quality Control (QC) and Quality Assurance (QA) systems. Laboratory Quality Control refers to the set of procedures used to monitor the analytical process and detect errors before patient results are released. It ensures precision and consistency in test performance, while Quality Assurance is a broader system that includes all planned activities from specimen collection to result reporting aimed at ensuring quality laboratory output (Oncohema Key, 2016). In haematology laboratories, QA typically includes Internal Quality Control (IQC), External Quality Assessment (EQA), equipment calibration, staff competency evaluation, and adherence to Standard Operating Procedures (SOPs) (Westgard, 2019).
Internal Quality Control involves daily monitoring of test procedures using control samples to ensure consistency, while External Quality Assessment compares laboratory performance with external reference standards or peer laboratories to ensure accuracy and harmonization of results across institutions (WHO, 2016). Studies have shown that laboratories that implement both IQC and EQA systems consistently produce more reliable and clinically acceptable results than those without structured quality systems (Shah et al., 2020).
In Nigeria, especially in large urban centres like Lagos State, teaching hospitals serve as referral centres where complex diagnostic tests are performed. These laboratories handle high patient volumes and a wide range of investigations, making quality control practices even more critical. However, previous studies have reported significant challenges in laboratory quality systems in developing countries, including inadequate staffing, poor equipment maintenance, irregular participation in external quality assessment schemes, lack of standardized procedures, and limited accreditation status (Adewoyin et al., 2019).
Despite improvements in laboratory medicine, inconsistencies in test results still occur in many healthcare facilities due to gaps in quality control practices. These inconsistencies can lead to misdiagnosis, inappropriate treatment, increased healthcare costs, and poor patient outcomes. Therefore, maintaining strict laboratory quality control practices is essential for ensuring patient safety and clinical accuracy.
In Lagos State, teaching hospitals such as Lagos University Teaching Hospital (LUTH), Lagos State University Teaching Hospital (LASUTH), and other federal or state tertiary health institutions provide advanced haematology diagnostic services. However, there is limited recent empirical data on how effectively quality control practices are being implemented in their haematology departments. Assessing these practices is therefore necessary to identify strengths, gaps, and areas requiring improvement in line with international laboratory quality standards.
This study therefore seeks to assess laboratory quality control practices in the haematology departments of selected teaching hospitals in Lagos State, Nigeria.
1.2 Statement of the Problem
Despite the critical role of haematology laboratories in clinical decision-making, concerns persist regarding the consistency, reliability, and accuracy of laboratory test results in many healthcare facilities in Nigeria. In teaching hospitals, where complex diagnostic services are expected to meet international standards, variations in test results and quality inconsistencies still occur, raising questions about the effectiveness of existing quality control systems.
Studies have shown that many clinical laboratories in developing countries struggle with implementing comprehensive quality management systems due to inadequate funding, insufficient trained personnel, lack of equipment calibration, and irregular participation in external quality assessment schemes. In Lagos State, previous assessments have revealed that only a small proportion of laboratories consistently adhere to international quality standards, and many lack formal accreditation or structured quality improvement programs (Adewoyin et al., 2019; UNILAG Research Report, 2019).
In haematology departments specifically, errors in test procedures such as haemoglobin estimation, packed cell volume (PCV), white blood cell counts, and platelet analysis can significantly affect patient diagnosis and treatment. These errors may arise from pre-analytical, analytical, or post-analytical stages of laboratory processing. However, in many teaching hospitals, quality control practices such as routine use of control materials, documentation of QC results, and participation in external quality assurance programs are either inconsistent or not fully implemented.
Furthermore, there is limited updated and comprehensive data on the current status of laboratory quality control practices in haematology departments of teaching hospitals in Lagos State. Most available studies are outdated or focus broadly on clinical laboratories without specifically examining haematology units. This creates a knowledge gap that limits evidence-based decision-making for laboratory quality improvement.
The absence of standardized and well-monitored quality control systems can lead to inaccurate test results, misdiagnosis, delayed treatment, and increased patient morbidity and mortality. Therefore, there is a need to critically assess the existing laboratory quality control practices in haematology departments of selected teaching hospitals in Lagos State to determine their level of compliance with international standards and identify areas requiring improvement.
1.3 Objectives of the Study
The main objective of this study is to assess laboratory quality control practices in the haematology departments of selected teaching hospitals in Lagos State, Nigeria.
Specific objectives are to:
- Determine the extent of implementation of internal quality control practices in haematology departments.
- Assess the level of participation in external quality assessment schemes among selected teaching hospitals.
- Evaluate the availability and use of standard operating procedures (SOPs) in haematology laboratories.
- Identify challenges affecting effective quality control implementation in haematology departments.
1.4 Research Questions

1.5 Research Hypothesis
H₀: There is no significant relationship between quality control practices and accuracy of haematology test results in selected teaching hospitals in Lagos State.
H₁: There is a significant relationship between quality control practices and accuracy of haematology test results in selected teaching hospitals in Lagos State.
1.6 Significance of the Study
This study will be beneficial to laboratory scientists, hospital management, and healthcare policymakers. It will provide insight into the current state of quality control practices in haematology laboratories and highlight areas that require improvement. For laboratory personnel, the study will reinforce the importance of adherence to quality control protocols. For hospital administrators, it will provide evidence to support investment in laboratory infrastructure, training, and accreditation. Policymakers will also benefit by using the findings to strengthen national laboratory quality assurance frameworks.
1.7 Scope of the Study
This study is limited to the haematology departments of selected teaching hospitals in Lagos State, Nigeria. It focuses on internal quality control practices, external quality assessment participation, use of standard operating procedures, and challenges affecting quality control implementation.
