INTRODUCTION
1.1 Background of the Study
Urinary Tract Infection (UTI) is one of the most common bacterial infections affecting humans globally and remains a significant cause of morbidity across all age groups. It refers to the invasion and multiplication of microorganisms, usually bacteria, in any part of the urinary system including the urethra, bladder, ureters, or kidneys. UTIs are among the leading causes of hospital visits and account for a substantial proportion of antimicrobial prescriptions worldwide, especially in developing countries where diagnostic facilities and antimicrobial stewardship are often inadequate (World Health Organization, 2023).
The burden of UTI is particularly high in sub-Saharan Africa, where environmental, socio-economic, and healthcare-related factors contribute to increased susceptibility and poor management outcomes. In Nigeria, UTIs represent a major public health concern due to limited access to quality diagnostic services, self-medication practices, and the widespread use of empirical antibiotic therapy without culture confirmation. These challenges have contributed significantly to the increasing prevalence of antibiotic-resistant uropathogens in many clinical settings (Okeke et al., 2020).
UTIs are predominantly caused by bacteria, with Escherichia coli accounting for the majority of community-acquired infections. Other common uropathogens include Klebsiella spp., Proteus spp., Pseudomonas aeruginosa, and Staphylococcus aureus. These organisms vary in their virulence and resistance patterns depending on geographical location, hospital environment, patient population, and antibiotic usage patterns (Nicolle, 2019).
A major concern in the management of UTIs is the increasing rate of antimicrobial resistance (AMR) among isolated pathogens. Studies across Nigeria have reported high resistance rates of uropathogens to commonly used antibiotics such as ampicillin, cotrimoxazole, and amoxicillin, while relatively better susceptibility has been observed with fluoroquinolones and aminoglycosides in some regions (Omoregie et al., 2010). Despite this, there is limited recent data on the prevalence of UTI and the antibiotic sensitivity patterns of bacterial isolates in this region, particularly across different hospital settings.

Without updated and region-specific data, clinicians may rely on outdated treatment guidelines, leading to poor clinical outcomes and increased healthcare burden.
Therefore, assessing the prevalence of UTI and the antibiotic sensitivity pattern of isolated bacteria among patients attending selected hospitals in Enugu State is essential for improving diagnosis, guiding appropriate antibiotic therapy, and strengthening infection control strategies.
1.2 Statement of the Problem
Urinary Tract Infection remains one of the most frequently diagnosed infections in clinical practice, yet it continues to be complicated by misdiagnosis, inappropriate treatment, and increasing antimicrobial resistance. In many hospitals in Nigeria, including those in Enugu State, UTI is often diagnosed based on clinical symptoms alone without laboratory confirmation, leading to frequent overuse or misuse of antibiotics.
This empirical approach to treatment has contributed significantly to the rising prevalence of antibiotic-resistant uropathogens. Studies have shown that many bacterial isolates responsible for UTI in Nigeria are resistant to commonly prescribed antibiotics, thereby limiting treatment options and increasing the risk of complications such as pyelonephritis, renal damage, and recurrent infections (Omoregie et al., 2010; Okeke et al., 2020).
Furthermore, there is inadequate and outdated data on the current prevalence of UTI and antibiotic sensitivity patterns of bacterial isolates in Enugu State. Most available studies are either hospital-specific, limited in scope, or no longer reflective of the current resistance trends. This creates a major gap in knowledge that affects clinical decision-making and public health planning.
In addition, routine urine culture and sensitivity testing are not consistently performed in many healthcare facilities due to cost, time constraints, and lack of laboratory infrastructure. As a result, clinicians often rely on empirical antibiotic therapy, which may not be effective against resistant strains circulating within the community and hospital environments.
The absence of updated regional surveillance data on uropathogens and their resistance patterns contributes to poor treatment outcomes, increased healthcare costs, prolonged hospital stays, and higher risk of complications among patients with UTI. Without current evidence, it becomes difficult to formulate effective antibiotic policies or implement targeted infection control measures.
Therefore, there is a need to evaluate the prevalence of urinary tract infection and the antibiotic sensitivity pattern of isolated bacteria among patients attending selected hospitals in Enugu State, Nigeria, in order to generate updated data that will guide effective treatment and antibiotic stewardship.
1.3 Objectives of the Study
The main objective of this study is to determine the prevalence of urinary tract infection and the antibiotic sensitivity pattern of isolated bacteria among patients attending selected hospitals in Enugu State, Nigeria.
Specific objectives are to:
- Determine the prevalence of urinary tract infection among patients attending selected hospitals in Enugu State.
- Identify the bacterial organisms responsible for UTI among the patients.
- Determine the antibiotic sensitivity pattern of the isolated bacterial organisms.
- Assess the most effective antibiotics for the treatment of UTI in the study area.
1.4 Research Questions
- What is the prevalence of urinary tract infection among patients in selected hospitals in Enugu State?

1.5 Research Hypothesis
H₀: There is no significant relationship between bacterial isolates and their antibiotic sensitivity patterns among patients with urinary tract infection in selected hospitals in Enugu State.
H₁: There is a significant relationship between bacterial isolates and their antibiotic sensitivity patterns among patients with urinary tract infection in selected hospitals in Enugu State.
1.6 Significance of the Study
This study will be beneficial to medical laboratory scientists, clinicians, hospital administrators, and public health policymakers. It will provide updated data on the prevalence of UTI and the antibiotic sensitivity pattern of uropathogens in Enugu State. This will guide clinicians in selecting appropriate empirical therapy and reduce treatment failures.
For laboratory personnel, the study will emphasize the importance of routine culture and sensitivity testing in the diagnosis of UTI. For hospital management and policymakers, the findings will support the development of antibiotic stewardship programs and infection control policies aimed at reducing antimicrobial resistance. The study will also contribute to existing academic literature on urinary tract infections in Nigeria.
1.7 Scope of the Study
This study is limited to patients attending selected hospitals in Enugu State, Nigeria. It focuses on the prevalence of urinary tract infection, identification of bacterial isolates, and determination of their antibiotic sensitivity patterns using standard laboratory methods.
