INTRODUCTION
1.1 Background of the Study
The advancement of digital technology and internet accessibility has transformed the way people communicate, access information, and make health-related decisions across the world (Castells, 2010). Social media platforms such as Facebook, WhatsApp, Instagram, TikTok, X (formerly Twitter), and YouTube have become major sources of information dissemination among young people (Kaplan & Haenlein, 2010). These platforms provide users with instant access to news, opinions, and health-related content (Boyd & Ellison, 2007). While social media has contributed positively to public health awareness and communication, it has also become a major channel for the spread of misinformation, especially concerning vaccines and immunization programs (WHO, 2023).
Health misinformation refers to false, inaccurate, misleading, or unverified health-related information shared intentionally or unintentionally through various communication channels (Wardle & Derakhshan, 2017). In recent years, vaccine-related misinformation on social media has become a major public health concern globally (WHO, 2023). False claims about vaccine safety, side effects, infertility, microchips, death risks, and conspiracy theories have contributed significantly to vaccine hesitancy and reduced public trust in healthcare systems (UNICEF, 2022). According to the World Health Organization (WHO, 2023), misinformation and vaccine hesitancy are among the leading threats to global public health because they undermine efforts to prevent infectious diseases through immunization.
Vaccination remains one of the most effective and cost-efficient public health interventions for preventing infectious diseases and reducing mortality rates (CDC, 2021). Vaccines have successfully controlled diseases such as polio, measles, hepatitis, tetanus, and COVID-19 (WHO, 2023). Despite the proven benefits of vaccines, many individuals continue to reject or delay vaccination due to fear, misconceptions, and misleading information obtained from social media platforms (UNICEF, 2022).
In Nigeria, social media usage has increased rapidly among young adults due to widespread smartphone ownership and internet penetration (NCC, 2023). Young adults frequently rely on social media for information because of its convenience, accessibility, and interactive nature (Kaplan & Haenlein, 2010). However, the uncontrolled spread of health misinformation on these platforms has negatively influenced public attitudes toward vaccination (WHO, 2023). During the COVID-19 pandemic, numerous false claims circulated online regarding vaccine safety and effectiveness (UNICEF, 2022). Some social media users claimed that vaccines caused infertility, altered human DNA, or were tools for population control. These misleading narratives created fear, distrust, and confusion among many Nigerians, especially young adults (WHO, 2023).
Ajah, a rapidly developing area in Lagos State, has a large population of educated and digitally active young adults who are highly exposed to social media content daily (National Population Commission, 2023). Due to increasing internet usage in the area, residents frequently encounter both accurate and misleading health information online. Many young adults in Ajah depend heavily on social media platforms for news and health updates instead of consulting healthcare professionals or official health agencies (UNICEF, 2022). This situation has raised concerns regarding the influence of social media misinformation on vaccine acceptance and public health behavior within the community.
Vaccine acceptance refers to the willingness of individuals to receive recommended vaccines when available (WHO, 2023). Acceptance of vaccines is influenced by several factors including education, religious beliefs, cultural perceptions, trust in government, perceived vaccine safety, and exposure to health information (Larson et al., 2014). Social media misinformation can significantly alter perceptions and attitudes toward vaccines by promoting fear and skepticism (Wardle & Derakhshan, 2017). Exposure to repeated false information may reduce confidence in vaccines and discourage individuals from participating in immunization programs (UNICEF, 2022).
Studies have shown that misinformation spreads faster on social media than verified information due to emotional content, sensational headlines, and peer influence (Vosoughi, Roy, & Aral, 2018). Young adults are particularly vulnerable because they spend significant time online and are more likely to share unverified information among peers (Kaplan & Haenlein, 2010). According to UNICEF (2022), misinformation targeting young populations has contributed to declining trust in immunization campaigns in several developing countries, including Nigeria.
The issue of vaccine hesitancy among young adults presents serious public health challenges because low vaccine uptake increases the risk of disease outbreaks and reduces herd immunity within communities (WHO, 2023). Public health experts have emphasized the need to understand how social media misinformation influences vaccine decisions in order to develop effective communication strategies and restore public trust in vaccination programs (CDC, 2021).
Although several studies have examined vaccine hesitancy in Nigeria, limited research has focused specifically on the effect of social media health misinformation on vaccine acceptance among young adults in Ajah. Considering the increasing influence of social media on health behavior, there is a need for empirical investigation into how online misinformation affects vaccine perceptions and decisions among young adults in the study area.
This study therefore seeks to examine the effect of social media health misinformation on vaccine acceptance among young adults in Ajah. The findings of the study are expected to provide useful information for public health practitioners, policymakers, and healthcare organizations in developing strategies to combat misinformation and improve vaccine acceptance.
1.2 Statement of the Problem
The widespread use of social media among young adults has significantly changed the way health information is accessed and shared. While social media platforms provide opportunities for rapid dissemination of public health information, they have also become major channels for the spread of false and misleading health information. In recent years, vaccine misinformation has increased considerably on social media platforms, leading to fear, confusion, mistrust, and vaccine hesitancy among many individuals.
In Nigeria, especially after the COVID-19 pandemic, social media became flooded with misinformation regarding vaccine safety, side effects, effectiveness, and hidden intentions behind vaccination campaigns. False narratives such as vaccines causing infertility, death, genetic mutation, or spiritual manipulation gained popularity online and influenced public perceptions negatively. Many young adults who rely heavily on social media for information were exposed to these misleading claims without verifying their authenticity.
Ajah, Lagos State, has a growing population of internet-savvy young adults who frequently engage with social media content. Despite public health campaigns promoting vaccination, there appears to be increasing skepticism and reluctance toward vaccine acceptance among some young adults in the area. Many individuals now question the safety and credibility of vaccines due to misinformation circulated online. This situation poses a serious threat to public health because reduced vaccine acceptance can increase the spread of preventable diseases and weaken community immunity.
The persistence of vaccine hesitancy among young adults may undermine government immunization efforts and increase the risk of future disease outbreaks. Furthermore, poor vaccine acceptance can result in increased healthcare costs, loss of productivity, and avoidable illness and deaths within communities.
Although previous studies have explored vaccine hesitancy and health communication in Nigeria, there is insufficient empirical evidence on the specific influence of social media health misinformation on vaccine acceptance among young adults in Ajah. Most existing studies focus on general public attitudes without paying adequate attention to the role of social media misinformation among urban youth populations.
Therefore, the problem this study seeks to address is the growing exposure of young adults in Ajah to social media health misinformation and how this exposure affects their willingness to accept vaccines. The study aims to provide evidence-based findings that may assist health authorities and policymakers in designing effective interventions to reduce misinformation and improve vaccine confidence among young adults.
1.3 Objectives of the Study
The main objective of this study is to examine the effect of social media health misinformation on vaccine acceptance among young adults in Ajah.
The specific objectives are to:
- determine the level of exposure of young adults in Ajah to health misinformation on social media;
- examine the influence of social media misinformation on attitudes toward vaccines among young adults;
- assess the relationship between social media misinformation and vaccine acceptance among young adults in Ajah;
- identify strategies for combating social media health misinformation and improving vaccine acceptance among young adults.
1.4 Research Questions
The following research questions will guide the study:
- What is the level of exposure of young adults in Ajah to health misinformation on social media?
- How does social media misinformation influence attitudes toward vaccines among young adults?
- What relationship exists between social media misinformation and vaccine acceptance among young adults in Ajah?
- What strategies can be used to combat social media health misinformation and improve vaccine acceptance?
1.5 Research Hypothesis
H₀: There is no significant relationship between social media health misinformation and vaccine acceptance among young adults in Ajah.
1.6 Significance of the Study
The study will be beneficial to public health practitioners by providing information on how misinformation affects vaccine-related decisions among young adults. The findings may help health professionals design effective communication strategies to improve public confidence in vaccines.
The study will also benefit government agencies and policymakers by identifying the extent to which social media misinformation contributes to vaccine hesitancy. The findings may guide the development of policies aimed at regulating false health information online and strengthening public health education campaigns.
Healthcare organizations and non-governmental organizations will benefit from the study by gaining insight into effective approaches for countering misinformation and promoting accurate vaccine information among young adults.
Academically, the study will contribute to existing literature on public health communication, vaccine hesitancy, and digital misinformation in Nigeria. It will also serve as a reference material for future researchers interested in related topics.
Finally, the study may help young adults become more aware of the dangers of misinformation and encourage informed decision-making regarding vaccination.
1.7 Scope of the Study
This study focuses on the effect of social media health misinformation on vaccine acceptance among young adults in Ajah, Lagos State. The study will specifically examine exposure to misinformation, attitudes toward vaccines, the relationship between misinformation and vaccine acceptance, and possible strategies for improving vaccine confidence among young adults within the study area.
1.8 Operational Definition of Terms
Social Media:
Digital platforms that enable users to create, share, and exchange information and content online.
Health Misinformation:
False, inaccurate, or misleading health-related information shared through various communication channels.
Vaccine Acceptance:
The willingness of individuals to receive recommended vaccines when available.
Vaccine Hesitancy:
