INTRODUCTION
1.1 Background of the Study
Family planning remains a fundamental component of global public health strategies aimed at improving maternal health, child survival, gender equality, and socioeconomic development. It enables individuals and couples to determine the number and spacing of their children through the voluntary use of contraceptive methods and informed reproductive choices. The importance of family planning has been emphasized globally because it contributes to the reduction of unintended pregnancies, maternal and infant mortality, and household economic pressures. According to the World Health Organization (WHO, 2023), access to quality family planning services allows individuals to exercise reproductive rights while improving health outcomes for women, children, and families.
In many developing countries, particularly in sub-Saharan Africa, family planning utilization remains influenced by social, economic, cultural, and healthcare system factors. Although contraceptive services have expanded through government and non-governmental interventions, challenges related to accessibility, affordability, misconceptions, and gender dynamics continue to affect effective utilization. The United Nations Population Fund (UNFPA, 2022) noted that financial barriers and unequal access to reproductive health services remain major obstacles preventing many couples from achieving their desired fertility goals.
Male participation in family planning has increasingly been recognized as an essential factor in improving reproductive health outcomes. Traditionally, family planning programmes have focused primarily on women, despite the fact that reproductive decisions often occur within marital relationships. Men frequently influence decisions regarding contraceptive adoption, family size preferences, financial support for healthcare services, and healthcare-seeking behaviour. According to Hardee, Croce-Galis, and Gay (2017), engaging men in family planning programmes improves contraceptive acceptance, enhances communication between partners, and promotes shared responsibility in reproductive decision-making.
Male involvement in family planning encompasses various forms of participation, including discussing reproductive choices with partners, supporting contraceptive use, accompanying spouses to healthcare facilities, providing financial resources, and making joint decisions regarding fertility preferences. However, in many African societies, cultural expectations regarding masculinity and gender roles often position men as primary decision-makers within households while limiting women’s autonomy in reproductive matters. This social structure makes men’s attitudes and participation critical determinants of family planning outcomes (Adelekan, Edoni, & Odimegwu, 2014).
Despite the recognized importance of male involvement, several barriers continue to limit men’s active participation in family planning. These barriers include inadequate reproductive health knowledge, misconceptions about contraceptives, religious and cultural beliefs, limited access to male-friendly reproductive health services, and economic challenges. Among these factors, financial accessibility remains a significant concern because healthcare costs directly affect whether couples can obtain and continue using family planning services (Cleland, Ali, & Benova, 2019).
Health insurance coverage has emerged as an important mechanism for reducing financial barriers to healthcare access. Health insurance provides financial protection by reducing the direct costs individuals pay when accessing healthcare services. Through insurance coverage, individuals and households are better able to utilize essential health services without experiencing excessive financial hardship. The World Bank (2022) identified financial protection as a major component of universal health coverage, emphasizing that health insurance systems can improve healthcare utilization among vulnerable populations.
In Nigeria, healthcare financing remains heavily dependent on out-of-pocket payment, where individuals pay directly for healthcare services at the point of use. This payment system creates significant barriers, especially among low-income households in rural communities. According to Onwujekwe et al. (2019), high reliance on out-of-pocket healthcare financing contributes to delayed healthcare utilization, reduced access to essential services, and increased financial burden among Nigerian households. Family planning services may also be affected because couples experiencing economic difficulties may prioritize other household needs over reproductive healthcare.
The introduction of health insurance schemes in Nigeria, including the National Health Insurance Authority (NHIA) initiatives, represents an effort to improve healthcare affordability and accessibility. Health insurance coverage has the potential to increase utilization of preventive and reproductive healthcare services by reducing direct payment requirements. However, coverage remains uneven, particularly among rural populations where awareness, enrollment, and accessibility challenges persist (Aregbeyen & Akinlo, 2021).
Rural communities often experience greater healthcare challenges compared to urban areas due to limited healthcare facilities, transportation difficulties, poverty, and lower awareness of health insurance programmes. These challenges may affect male participation in family planning because men may be less willing or unable to support services that require additional financial commitments. According to Gakidou et al. (2017), socioeconomic inequalities significantly influence access to healthcare services in low- and middle-income countries.
The relationship between health insurance coverage and male participation in family planning is therefore important because financial protection may influence men’s willingness and ability to support reproductive health decisions. When couples have access to affordable healthcare through insurance schemes, men may be more likely to encourage contraceptive use, accompany their partners to healthcare facilities, and participate actively in fertility discussions. Conversely, absence of insurance coverage may increase financial concerns and discourage male involvement.
Evidence from other developing countries suggests that health insurance coverage improves healthcare utilization and reduces financial barriers to reproductive health services. However, the extent to which insurance coverage influences male participation in family planning decisions remains less explored, particularly within Nigerian rural communities. Most existing studies have concentrated on women’s contraceptive use, maternal healthcare utilization, and general health insurance outcomes, leaving limited understanding of how insurance affects men’s involvement in reproductive decision-making.
Asaba, the capital city of Delta State, Nigeria, contains both urban and surrounding rural communities where reproductive health practices are influenced by socioeconomic conditions, cultural beliefs, and healthcare accessibility. Selected rural communities around Asaba provide an important context for examining how health insurance coverage influences male participation in family planning decisions. Understanding this relationship may provide useful evidence for improving reproductive health programmes and strengthening male engagement strategies.
Therefore, this study seeks to examine the influence of health insurance coverage on male participation in family planning decisions among married couples in selected rural communities in Asaba. The study focuses on how insurance availability, affordability, and accessibility affect men’s involvement in contraceptive decision-making, financial support, and shared reproductive responsibilities.
1.2 Statement of the Problem
Family planning remains a critical intervention for improving maternal health, reducing unintended pregnancies, and promoting household welfare. However, despite continuous efforts by government agencies and healthcare organizations to increase contraceptive access in Nigeria, utilization remains below desired levels, particularly in rural communities. One of the major challenges affecting family planning outcomes is inadequate male participation in reproductive health decisions.
Although women are often the primary users of contraceptive methods, men play significant roles in determining whether family planning services are accepted, financed, and sustained within households. In many Nigerian communities, men control household financial resources and influence reproductive decisions. Therefore, limited male participation may negatively affect contraceptive adoption, continuation, and effective family planning practices.
A major factor contributing to low male participation is the financial burden associated with accessing healthcare services. Many Nigerian households rely on direct payment for healthcare expenses, which may discourage couples from seeking reproductive health services. When healthcare costs compete with other household needs, men may become less supportive of family planning due to concerns about affordability and economic priorities.
Health insurance coverage has the potential to address these challenges by reducing direct healthcare expenses and improving access to reproductive health services. However, despite the expansion of health insurance initiatives in Nigeria, many rural households remain excluded due to limited awareness, affordability challenges, and inadequate coverage. Consequently, the potential contribution of health insurance towards improving male participation in family planning remains uncertain.
Existing studies have examined health insurance coverage in relation to healthcare utilization, maternal health services, and financial protection, but limited attention has been given to its influence on male involvement in family planning decisions. Furthermore, many reproductive health studies have focused primarily on women’s experiences, creating a knowledge gap regarding men’s perspectives and participation.
The contextual gap is also significant because rural communities around Asaba experience unique socioeconomic and healthcare accessibility challenges. Factors such as income level, availability of insurance schemes, cultural expectations, and healthcare accessibility may influence whether men actively participate in family planning decisions.
Therefore, the problem of this study is that despite the importance of male involvement in achieving successful family planning outcomes, limited empirical evidence exists on how health insurance coverage influences male participation among married couples in rural communities in Asaba. This study seeks to address this gap by examining whether health insurance coverage contributes to increased male involvement in family planning decisions among married couples.
1.3 Aim of the Study
The main aim of this study is to examine the influence of health insurance coverage on male participation in family planning decisions among married couples in selected rural communities in Asaba.
1.3.1 Objectives of the Study
The specific objectives are to:
- Examine the influence of availability of health insurance coverage on male participation in family planning decisions among married couples in selected rural communities in Asaba.
- Determine the extent to which affordability of health insurance services affects male support for family planning practices among married couples.
- Assess the influence of accessibility to health insurance programmes on men’s involvement in contraceptive decision-making among couples.
- Examine the relationship between health insurance coverage and financial support for family planning services among married couples.
1.4 Research Questions
The study seeks to answer the following questions:
- To what extent does availability of health insurance coverage influence male participation in family planning decisions among married couples?
- How does affordability of health insurance services affect male support for family planning practices?
- What influence does accessibility to health insurance programmes have on men’s involvement in contraceptive decision-making?
- What relationship exists between health insurance coverage and financial support for family planning services among married couples?
1.5 Research Hypothesis
H₀: Health insurance coverage has no significant influence on male participation in family planning decisions among married couples in selected rural communities in Asaba.
1.6 Significance of the Study
This study is significant because it provides empirical evidence on the relationship between health insurance coverage and male participation in family planning decisions among married couples in selected rural communities in Asaba. The findings will contribute to the understanding of how financial protection mechanisms influence men’s willingness and ability to participate in reproductive health decisions.
The study will be beneficial to government agencies and policymakers, particularly those responsible for healthcare financing and reproductive health programmes. The findings will provide useful information on the importance of expanding health insurance coverage as a strategy for improving family planning outcomes. Evidence from the study may assist policymakers in designing interventions that increase insurance enrollment among rural households and reduce financial barriers to reproductive healthcare access.
The study will also be valuable to healthcare providers and reproductive health organizations. Healthcare workers, family planning coordinators, and community health practitioners may use the findings to develop strategies that encourage greater male engagement in reproductive health services. By understanding the relationship between insurance coverage and male involvement, healthcare providers can design programmes that actively include men in family planning counselling and decision-making processes.
Furthermore, the study will benefit married couples and families by highlighting the importance of shared responsibility in reproductive health decisions. Increased awareness of the role of financial protection and male involvement may encourage couples to communicate better, make informed fertility decisions, and utilize available family planning services effectively.
The study will also contribute to academic knowledge and future research by addressing an existing gap in reproductive health literature. While previous studies have examined health insurance coverage and healthcare utilization, limited attention has been given to how insurance influences male participation in family planning decisions. The findings will provide a foundation for future studies focusing on health financing, gender roles, and reproductive health behaviours.
Finally, the study will be useful to community leaders and non-governmental organizations (NGOs) involved in health promotion. The findings may guide community-based interventions aimed at increasing awareness of health insurance programmes and promoting male involvement in family planning practices.
1.7 Scope of the Study
This study focused on examining the influence of health insurance coverage on male participation in family planning decisions among married couples in selected rural communities in Asaba, Delta State, Nigeria.
The study was limited to selected rural communities within the Asaba area and involved married couples of reproductive age. The study examined health insurance coverage as the independent variable and male participation in family planning decisions as the dependent variable.
Specifically, the study focused on four dimensions of health insurance coverage:
- Availability of health insurance coverage and its influence on male participation in family planning decisions.
- Affordability of health insurance services and its effect on men’s support for family planning practices.
- Accessibility of health insurance programmes and its influence on men’s involvement in contraceptive decision-making.
- Health insurance coverage and financial support for family planning services among married couples.
The study did not examine other possible determinants of male participation such as religious beliefs, cultural practices, educational level, or gender norms except where they relate to the main variables under investigation.
1.8 Operational Definition of Terms
Health Insurance Coverage
Health insurance coverage refers to the extent to which individuals or households are enrolled in a health financing scheme that provides financial protection against healthcare expenses and improves access to essential health services.
Male Participation
Male participation refers to the active involvement of husbands or male partners in family planning activities, including discussing contraceptive choices, supporting healthcare decisions, providing financial assistance, and participating in reproductive decisions.
Family Planning
Family planning refers to the voluntary process through which individuals and couples determine the number, timing, and spacing of their children through the use of contraceptive methods and reproductive health information.
Family Planning Decisions
Family planning decisions refer to choices made by couples regarding contraceptive use, desired family size, birth spacing, and reproductive health practices.
Married Couples
Married couples refer to husbands and wives legally or traditionally recognized as partners who share reproductive decisions and family responsibilities.
Availability of Health Insurance
Availability of health insurance refers to the presence and accessibility of health insurance schemes or programmes that couples can enroll in to obtain financial protection for healthcare services.
