Chapter One: Introduction

1.1 Background to the Study

The persistent conflict between Fulani herdsmen and farming communities in Nigeria, particularly in Benue State, has escalated over the years, causing significant socio-economic and health-related challenges (Okoye, 2023). These clashes often arise from competition over land and water resources, which are essential for both crop farming and cattle grazing. The resultant disputes frequently lead to violent encounters, property destruction, and loss of lives, thereby destabilizing local communities and disrupting normal economic activities (Ojo, 2023).

In Logo Local Government Area (LGA), these conflicts have particularly disrupted agricultural activities, which form the backbone of the local economy. The destruction of farmlands and livestock has resulted in food insecurity and loss of livelihoods, leaving many families in precarious situations (Abdullahi, 2023). Consequently, households affected by these clashes experience economic hardship, which further limits their capacity to access healthcare services, purchase essential medicines, or seek preventive care (Adebayo, 2023).

The health sector in Logo LGA has also borne the brunt of these conflicts. Existing healthcare facilities, already constrained by limited resources and personnel, have been overwhelmed by the influx of internally displaced persons (IDPs) fleeing violence in surrounding communities (Okoye, 2023). This sudden increase in patient load has strained hospitals and clinics, leading to overcrowded wards, shortages of drugs, and delayed medical attention, thereby compromising the quality of healthcare delivery (Ojo, 2023).

Displaced populations often settle in temporary camps or with host families under substandard conditions, which exacerbates public health risks. Overcrowding, poor sanitation, and limited access to clean water in these settings contribute to the rapid spread of communicable diseases such as malaria, cholera, and respiratory infections (Abdullahi, 2023). Furthermore, psychological trauma resulting from violence, displacement, and loss of property heightens the burden on mental health services, which remain largely inadequate in the region (Adebayo, 2023).

Women and children, as the most vulnerable groups, are disproportionately affected by these disruptions. Pregnant women face difficulties accessing antenatal care and skilled birth attendants, while children often experience interruptions in routine immunizations and nutritional support (Okoye, 2023). These challenges increase the risk of maternal and infant morbidity and mortality, undermining progress toward achieving national health targets in Benue State (Ojo, 2023).

Moreover, the conflict undermines the effectiveness of public health interventions and disease control programs. Outreach campaigns for vaccination, malaria prevention, and maternal-child health often face logistical challenges in areas affected by violence, as healthcare workers fear for their safety and mobility is restricted (Abdullahi, 2023). Given these challenges, it becomes imperative to assess the specific impacts of Fulani herdsmen-farmer conflicts on healthcare delivery in Logo LGA. Understanding the nature and extent of these disruptions is essential for policymakers, health planners, and humanitarian organizations to develop targeted interventions that strengthen healthcare systems, improve access to essential services, and reduce the health vulnerabilities of affected populations (Abdullahi, 2023). This study, therefore, seeks to fill the gap in empirical research by examining the consequences of inter-communal conflicts on healthcare delivery in the region (Adebayo, 2023).

1.2 Statement of the Problem

Recurring clashes between Fulani herdsmen and farmers in Logo LGA have led to widespread displacement, destruction of property, and loss of lives, disrupting the social and economic fabric of local communities (Okoye, 2023). These conflicts often occur over access to farmland and grazing areas, with both sides claiming rightful ownership. The resulting violence has made many communities unsafe, forcing residents to flee their homes and seek refuge in temporary settlements or with relatives, thereby creating a humanitarian crisis (Ojo, 2023).

The influx of internally displaced persons (IDPs) into host communities has placed enormous pressure on local healthcare facilities. Hospitals and clinics, already operating with limited staff and resources, struggle to meet the increased demand for medical attention (Abdullahi, 2023). Shortages of essential drugs, medical equipment, and skilled health personnel exacerbate the situation, leading to long waiting times, insufficient care, and, in some cases, preventable deaths (Adebayo, 2023).

Displacement and overcrowding in temporary settlements have also contributed to public health risks. Poor sanitation, inadequate access to clean water, and insufficient shelter create conditions conducive to the spread of communicable diseases such as cholera, malaria, and respiratory infections (Abdullahi, 2023). Vulnerable populations, particularly children, pregnant women, and the elderly, are disproportionately affected, resulting in higher morbidity and mortality rates (Okoye, 2023).

The psychological impact of these conflicts is another pressing concern. This gap hinders evidence-based policymaking and the design of targeted interventions to support healthcare provision in conflict-affected areas (Okoye, 2023).

Therefore, assessing the impact of Fulani herdsmen-farmer conflicts on healthcare delivery in Logo LGA is critical. Understanding the scope of healthcare disruptions, the capacity of facilities to cope, and the specific challenges faced by healthcare providers can inform interventions aimed at improving health outcomes for both displaced and resident populations. This study seeks to provide empirical evidence that can guide policymakers, health planners, and humanitarian agencies in mitigating the health consequences of ongoing conflicts (Adebayo, 2023).

1.3 Objectives of the Study

The study aims to:

1. Assess the extent to which Fulani herdsmen-farmer conflicts affect healthcare delivery in Logo LGA.
2. Evaluate the capacity of existing healthcare facilities to meet the needs of displaced and host populations.
3. Identify challenges faced by healthcare providers in delivering services amidst ongoing conflicts.
4. Propose recommendations to mitigate the adverse effects of these conflicts on healthcare delivery.

1.5 Research Hypothesis

The research hypothesis is buttressed below:

H₀: Fulani herdsmen-farmer conflicts do not significantly affect healthcare delivery in Logo LGA.

H₁: Fulani herdsmen-farmer conflicts significantly affect healthcare delivery in Logo LGA.

1.6 Significance of the Study

The study is significant as it provides a comprehensive understanding of how inter-communal conflicts, particularly between Fulani herdsmen and farming communities, impact healthcare delivery in Logo Local Government Area. By focusing on healthcare, the research highlights the challenges faced by health facilities, the strain on medical personnel, and the increased vulnerability of displaced and host populations. 1.7 Scope of the Study

The study focuses on Logo Local Government Area in Benue State, Nigeria, covering the period from 2015 to 2025. It includes all healthcare facilities within the LGA, such as primary healthcare centers, general hospitals, and clinics, and assesses their capacity to provide services amidst the ongoing conflicts (RSI International, 2023).

1.8 Operational Definitions

Fulani Herdsmen: These are nomadic or semi-nomadic pastoralists, primarily of Fulani ethnicity, who are engaged in cattle rearing. Their movement in search of grazing land often brings them into contact and sometimes conflict with sedentary farming communities.

Farmer-Herder Conflict: This refers to disputes that arise between farming communities and pastoralists over access to land, water, and other natural resources. These conflicts are often characterized by violent encounters, destruction of property, and displacement of affected populations.

Healthcare Delivery: Healthcare delivery encompasses the provision of medical services to individuals and communities. This includes preventive care, diagnosis, treatment, rehabilitation, and emergency services aimed at maintaining or improving health and well-being.

Internally Displaced Persons (IDPs): IDPs are individuals or groups who have been forced to flee their homes due to violence, conflict, or natural disasters but remain within their country’s borders. Unlike refugees, they do not cross international boundaries but often require humanitarian assistance and healthcare support.

Logo Local Government Area (LGA): This is an administrative region in Benue State, Nigeria, affected by inter-communal conflicts. The area is predominantly agrarian, and its residents are highly dependent on both farming and local healthcare facilities for sustenance and well-being.

Conflict-Affected Communities: These are communities that experience disruptions to social, economic, and healthcare systems as a result of violent disputes. In this context, it refers to settlements in Logo LGA directly impacted by herdsmen-farmer clashes.

Healthcare Access: Healthcare access refers to the availability, affordability, and timeliness of health services for individuals within a community. In conflict settings, access can be severely limited due to insecurity, displacement, or overburdened facilities.

Health System Capacity: This term refers to the ability of healthcare facilities and providers to effectively meet the health needs of a population. It includes the availability of medical personnel, equipment, infrastructure, and supplies necessary to provide quality care.
