Project – Assessment of Knowledge, Attitudes and Practices Regarding Hypertension Prevention Among Adults in Selected Communities in Lagos State, Nigeria

CHAPTER ONE

INTRODUCTION

1.1 Background of the Study

Hypertension remains one of the most significant public health challenges globally and is a major contributor to cardiovascular diseases, stroke, kidney failure, and premature mortality. It is often described as a “silent killer” because many individuals live with elevated blood pressure for years without noticeable symptoms until severe complications occur. According to the World Health Organization (WHO), hypertension affects more than one billion adults worldwide and is responsible for millions of preventable deaths annually, particularly through its association with cardiovascular and cerebrovascular diseases (World Health Organization [WHO], 2023). Despite advances in medical treatment and increased awareness campaigns, hypertension prevalence continues to rise due to changing lifestyles, urbanization, unhealthy dietary patterns, physical inactivity, obesity, and inadequate preventive health practices.

Hypertension is defined as a persistent elevation of blood pressure, commonly characterized by systolic blood pressure of 140 mmHg or higher and/or diastolic blood pressure of 90 mmHg or higher. Although hypertension can be effectively prevented and controlled through lifestyle modification, early detection, and appropriate medical management, many affected individuals remain undiagnosed or inadequately managed (Unger et al., 2020). Prevention strategies include reducing dietary salt intake, maintaining a healthy body weight, engaging in regular physical activity, consuming fruits and vegetables, limiting alcohol intake, avoiding tobacco use, and undergoing regular blood pressure screening (WHO, 2023). However, the effectiveness of these preventive measures largely depends on individuals’ knowledge, attitudes, and daily health practices.

The burden of hypertension has become particularly concerning in low- and middle-income countries, including those in sub-Saharan Africa. Historically, hypertension was considered more prevalent in high-income countries, but rapid socioeconomic changes, urbanization, and lifestyle transitions have contributed to increasing rates in African populations. The Prospective Urban Rural Epidemiology (PURE) study demonstrated that hypertension prevalence is rising rapidly in developing countries due to increased exposure to risk factors associated with modernization and urban lifestyles (Chow et al., 2013). In many African countries, the challenge is compounded by limited access to healthcare services, inadequate screening programmes, poor awareness, and weak continuity of care.

Nigeria, as the most populous country in Africa, carries a substantial burden of hypertension. Studies have consistently shown that hypertension prevalence among Nigerian adults is high, with variations across geographical regions and population groups. A systematic review and meta-analysis by Adeloye et al. (2021) estimated that hypertension affects a significant proportion of Nigerian adults, highlighting the urgent need for effective prevention and control interventions. The increasing prevalence has been linked to demographic changes, dietary transitions, sedentary lifestyles, increasing obesity rates, and limited engagement in preventive health behaviours.

Lagos State represents a unique environment for examining hypertension prevention because it is Nigeria’s most urbanized and economically active state. The rapid urban expansion, population density, stressful work environments, changing dietary habits, and reduced physical activity associated with urban living have contributed to increased exposure to hypertension risk factors. Adults living in Lagos communities are increasingly adopting lifestyles characterized by consumption of processed foods, high salt intake, inadequate exercise, and prolonged occupational stress. These factors increase the likelihood of developing hypertension and related cardiovascular complications.

The prevention and control of hypertension require more than the availability of medical treatment; they depend significantly on community awareness and behavioural change. Knowledge of hypertension influences individuals’ ability to recognize risk factors, understand preventive measures, seek screening services, and comply with recommended lifestyle modifications. Individuals who possess adequate knowledge about hypertension are more likely to engage in healthier behaviours, including regular blood pressure monitoring and dietary adjustments (Kaliyaperumal et al., 2016). Conversely, inadequate knowledge may contribute to misconceptions, delayed diagnosis, poor health-seeking behaviour, and ineffective prevention practices.

Attitudes toward hypertension prevention also play an important role in determining health outcomes. Some individuals may recognize hypertension as a serious disease but fail to adopt preventive behaviours due to cultural beliefs, perceived lack of susceptibility, financial limitations, or misconceptions regarding the disease. The Health Belief Model suggests that individuals are more likely to engage in preventive actions when they perceive themselves as vulnerable to a health condition and believe that preventive measures are beneficial (Rosenstock et al., 1988). Therefore, assessing adults’ attitudes toward hypertension prevention provides insight into factors that may facilitate or hinder effective disease prevention.

Similarly, preventive practices among adults determine the extent to which knowledge and attitudes are translated into actual health behaviours. Practices such as regular blood pressure checks, healthy eating, physical exercise, weight management, and avoidance of harmful substances are essential for reducing hypertension risk. However, evidence from developing countries indicates that awareness of hypertension does not always translate into appropriate preventive practices. Many individuals may have heard about hypertension but continue to maintain behaviours that increase their risk of developing the disease (Guwatudde et al., 2015).

The Knowledge, Attitudes, and Practices (KAP) framework is widely used in public health research to evaluate community understanding, perceptions, and behaviours regarding disease prevention. The framework assumes that knowledge influences attitudes, while attitudes influence practices, although social and environmental factors may affect this relationship. Assessing KAP regarding hypertension prevention among adults provides valuable information for designing targeted health education programmes, strengthening community-based interventions, and improving cardiovascular disease prevention strategies.

In Lagos State, community-based approaches are particularly important because healthcare facilities alone cannot address the growing hypertension burden. Public health interventions focusing on awareness creation, lifestyle modification, routine screening, and improved access to preventive services are necessary to reduce hypertension-related morbidity and mortality. Understanding the current level of knowledge, attitudes, and preventive practices among adults in selected Lagos communities will help identify existing gaps and inform appropriate health promotion strategies.

Therefore, this study seeks to assess the knowledge, attitudes, and practices regarding hypertension prevention among adults in selected communities in Lagos State, Nigeria. The findings will contribute to understanding community-level factors influencing hypertension prevention and provide evidence for strengthening health education and preventive healthcare initiatives.

1.2 Statement of the Problem

Hypertension remains a growing public health concern in Nigeria despite continuous efforts by government agencies, healthcare professionals, and international organizations to promote cardiovascular health. The increasing prevalence of hypertension has resulted in rising cases of stroke, heart failure, kidney disease, and other complications that place significant pressure on individuals, families, and the healthcare system. Although hypertension is largely preventable through lifestyle modification and early detection, many Nigerian adults continue to experience poor prevention outcomes due to inadequate awareness, unhealthy behaviours, and limited utilization of preventive health services.

One major challenge in hypertension prevention is the gap between knowledge and practice among adults. Many individuals may have basic awareness that hypertension is associated with excessive salt intake, stress, obesity, and physical inactivity; however, this awareness does not always translate into consistent preventive behaviours. Some adults rarely check their blood pressure unless symptoms occur, while others continue unhealthy dietary and lifestyle practices despite knowing their potential health consequences. This gap suggests that knowledge alone may not be sufficient to achieve effective hypertension prevention.

Furthermore, misconceptions and negative attitudes toward hypertension contribute to delayed diagnosis and poor management. Some individuals perceive hypertension as a disease that affects only older people, while others believe that absence of symptoms indicates good health. Such perceptions may discourage early screening and preventive actions. Cultural beliefs, economic challenges, competing daily responsibilities, and limited access to health information may also influence attitudes toward hypertension prevention.

The urban environment of Lagos State presents additional challenges. While urbanization has improved access to healthcare facilities and health information, it has also encouraged lifestyles associated with increased hypertension risk. Many adults experience sedentary work patterns, high levels of stress, consumption of fast foods, and reduced opportunities for physical activity. These conditions create an environment where hypertension risk factors can easily develop and persist.

Although several studies have examined hypertension prevalence and risk factors in Nigeria, there remains insufficient community-specific evidence regarding the knowledge, attitudes, and practices of adults concerning hypertension prevention, particularly in selected communities in Lagos State. Without adequate understanding of these factors, health promotion interventions may fail to address the actual beliefs, behaviours, and challenges experienced by community members.

The absence of reliable information on community-level preventive behaviours creates difficulties for public health planners in designing effective hypertension prevention programmes. Therefore, assessing the knowledge, attitudes, and practices regarding hypertension prevention among adults in selected communities in Lagos State is necessary to identify existing gaps and provide evidence-based recommendations for improving cardiovascular health outcomes.
