INTRODUCTION

1.1 Background to the Study

Diabetes mellitus has emerged as one of the most significant non-communicable diseases globally, posing serious health, social, and economic challenges. It is a chronic metabolic disorder characterized by elevated blood glucose levels resulting from defects in insulin secretion, insulin action, or both. Over the past few decades, the prevalence of diabetes has increased dramatically, particularly in low- and middle-income countries such as Nigeria, where healthcare systems are often inadequately equipped to manage chronic diseases (World Health Organization [WHO], 2021).

The burden of diabetes is especially pronounced among aging populations. Aging is associated with physiological changes such as reduced insulin sensitivity, impaired glucose metabolism, and increased risk of obesity, all of which contribute to the development and progression of diabetes. In addition, older adults are more likely to suffer from multiple comorbidities such as hypertension, cardiovascular diseases, and kidney disorders, which complicate diabetes management and increase the risk of mortality (International Diabetes Federation [IDF], 2021).

In Nigeria, the growing number of elderly individuals combined with changing lifestyles, urbanization, and dietary transitions has contributed to a steady rise in diabetes cases. Kano State, one of the most populous states in Nigeria, is experiencing a similar trend. The increasing prevalence of diabetes among older adults in the state presents a significant public health concern, necessitating effective interventions to manage the disease and improve quality of life.

Public health interventions play a critical role in the prevention, control, and management of diabetes. These interventions include health education and awareness campaigns, early screening and diagnosis, lifestyle modification programs, provision of affordable medications, regular monitoring, and community-based support systems. Effective public health strategies not only reduce the incidence of diabetes but also minimize complications such as blindness, amputations, stroke, and kidney failure.

Healthcare institutions such as Aminu Kano Teaching Hospital serve as key centers for implementing these interventions. As a tertiary healthcare facility, the hospital provides specialized care for diabetes patients, including diagnostic services, treatment, counseling, and follow-up care. It also serves as a hub for public health programs aimed at improving diabetes management among aging populations.

Despite these efforts, challenges persist in the effective implementation and utilization of public health interventions. Limited healthcare resources, inadequate access to quality care, low health literacy, poverty, and cultural beliefs often hinder the success of diabetes management programs. Furthermore, many elderly patients face difficulties adhering to treatment regimens due to financial constraints, forgetfulness, and lack of social support.

Another important issue is the gap between policy formulation and implementation. While Nigeria has developed national strategies for non-communicable diseases, including diabetes, the actual delivery of these interventions at the grassroots level remains inconsistent. This gap is particularly evident in states like Kano, where disparities in healthcare access and service delivery continue to affect vulnerable populations, including the elderly.

Given the increasing burden of diabetes and the vulnerability of aging populations, it is essential to assess the effectiveness of existing public health interventions. Such an assessment will help identify strengths, weaknesses, and areas for improvement, thereby enhancing the overall management of diabetes.

This study, therefore, seeks to assess public health interventions in the management of diabetes among aging populations in Kano State, with a focus on Aminu Kano Teaching Hospital. The findings are expected to contribute to improved healthcare delivery and better health outcomes for elderly individuals living with diabetes.

1.2 Statement of the Problem

Diabetes mellitus has become a growing public health challenge in Nigeria, particularly among aging populations who are more susceptible to the disease and its complications. In Kano State, the increasing number of elderly individuals diagnosed with diabetes has placed a significant burden on both patients and healthcare systems.

Despite the presence of public health interventions aimed at managing diabetes—such as health education programs, screening initiatives, and access to treatment—there is evidence that many elderly patients continue to experience poor health outcomes. Complications such as diabetic foot ulcers, vision impairment, cardiovascular diseases, and kidney failure remain prevalent, suggesting that existing interventions may not be fully effective.

One major concern is the limited accessibility and utilization of these interventions. Many elderly individuals in Kano State face barriers such as financial constraints, inadequate healthcare infrastructure, long distances to healthcare facilities, and low awareness of available services. These challenges reduce their ability to benefit from public health programs designed to manage diabetes.

Additionally, there is a lack of comprehensive evaluation of these interventions, particularly in relation to their effectiveness among aging populations. While several programs have been implemented, there is insufficient empirical data assessing how well these interventions address the unique needs of elderly patients.

At Aminu Kano Teaching Hospital, which serves as a major referral center, there is a noticeable increase in the number of elderly diabetes patients seeking care. However, issues such as overcrowding, limited healthcare personnel, and resource constraints may affect the quality and effectiveness of care provided.

Furthermore, cultural beliefs, low health literacy, and poor adherence to treatment regimens among elderly patients continue to hinder successful diabetes management. Without proper assessment, it is difficult to determine whether current public health strategies are achieving their intended goals.

Therefore, this study seeks to address these gaps by assessing the effectiveness of public health interventions in the management of diabetes among aging populations in Kano State, using Aminu Kano Teaching Hospital as a case study. The study aims to provide evidence-based recommendations for improving diabetes care and enhancing the well-being of elderly patients.

1.3 Research Questions

1. What public health interventions are available for the management of diabetes among aging populations in Kano State?

2. How effective are these interventions in improving the health outcomes of elderly diabetes patients?

3. What challenges affect the implementation and utilization of public health interventions for diabetes management?

4. How can public health interventions be improved to better serve aging populations with diabetes?

1.4 Objectives of the Study

The main objective of this study is to assess public health interventions in the management of diabetes among aging populations in Kano State.

The specific objectives are to:

1. Identify existing public health interventions for diabetes management in Kano State.

2. Evaluate the effectiveness of these interventions among elderly patients.

3. Examine the challenges affecting the implementation and utilization of these interventions.

4. Suggest strategies for improving diabetes management among aging populations.

1.5 Research Hypothesis

H₀ : Public health interventions have no significant effect on the management of diabetes among aging populations in Kano State.

H₁ : Public health interventions have a significant effect on the management of diabetes among aging populations in Kano State.

1.6 Significance of the Study

This study is significant as it will provide valuable insights into the effectiveness of public health interventions in managing diabetes among elderly populations. It will contribute to academic literature on non-communicable diseases and public health in Nigeria.

The findings will be beneficial to policymakers and health authorities in designing more effective and targeted interventions for diabetes management. Healthcare providers at Aminu Kano Teaching Hospital and other institutions will also benefit from the study by gaining a better understanding of the challenges faced by elderly patients.

Furthermore, the study will help raise awareness about the importance of effective diabetes management and encourage improved health-seeking behavior among aging populations.

1.7 Scope of the Study

This study focuses on the assessment of public health interventions in the management of diabetes among aging populations in Kano State. It specifically examines elderly patients receiving care at Aminu Kano Teaching Hospital.

The study covers key intervention areas such as health education, screening programs, access to medication, lifestyle modification, and follow-up care. It also considers the challenges affecting the implementation and effectiveness of these interventions.

1.8 Operational Definition of Terms

• Diabetes Mellitus: A chronic metabolic disorder characterized by high blood glucose levels.

• Public Health Interventions: Organized efforts and programs designed to prevent disease and promote health at the population level.

• Aging Population: Individuals aged 60 years and above.

• Disease Management: The process of controlling a disease to reduce its impact on health.

• Health Outcomes: The results of healthcare interventions on a patient’s health status.
