INTRODUCTION
1.1 Background of the Study
Health Information Systems (HIS) are critical for efficient healthcare delivery as they provide mechanisms for collecting, storing, analyzing, and disseminating health data that inform decision-making and policy formulation (World Health Organization [WHO], 2016). In primary health care (PHC) settings, HIS facilitates patient record-keeping, disease surveillance, resource allocation, and reporting of health indicators, which are essential for achieving universal health coverage (Awoonor-Williams et al., 2021). Despite their importance, HIS implementation in developing countries, including Nigeria, faces considerable challenges, especially in resource-constrained PHC facilities (Olajide et al., 2020).
PHCs in Nigeria serve as the first point of contact for the majority of citizens and are intended to provide essential promotive, preventive, curative, and rehabilitative services at the community level. However, these facilities are often underfunded, poorly staffed, and inadequately equipped, which hampers the effective deployment of HIS (Fatusi & Hindin, 2010). Studies indicate that less than half of public PHC facilities in Nigeria meet the minimum compliance standards for infrastructure, staffing, and essential medicines (National Primary Health Care Development Agency [NPHCDA], 2019). Such structural and operational weaknesses significantly impede the use of HIS for effective health service delivery.
Furthermore, the successful adoption of HIS in PHCs is influenced by factors such as staff training, availability of technological infrastructure, organizational support, and funding (Ojo et al., 2022). Poor training of healthcare workers in data management and lack of ongoing professional development results in low proficiency in handling electronic health records and other HIS platforms. Similarly, unreliable electricity, inadequate ICT infrastructure, and limited internet connectivity further restrict the functionality and reliability of HIS in PHCs (WHO, 2016).
Implementing HIS is therefore a multifaceted challenge requiring not just technology but also strong institutional support, skilled personnel, and adequate funding (Awoonor-Williams et al., 2021). Lokoja, the capital of Kogi State, represents a typical context in which PHCs operate under resource constraints while trying to maintain efficient health data management systems. Understanding the challenges of HIS implementation in selected PHCs in Lokoja is crucial for improving data quality, enhancing service delivery, and strengthening the overall health system at the community level (Olajide et al., 2020).
1.2 Statement of the Problem
Despite the central role of PHCs in providing essential health services, many facilities in Nigeria, including those in Lokoja, struggle with inefficient or incomplete HIS (Fatusi & Hindin, 2010). Inadequate data collection, delays in reporting, poor data accuracy, and weak integration of health information into decision-making hinder the effectiveness of PHCs. Key challenges include insufficient technical infrastructure, limited training of healthcare workers, inadequate funding, and poor organizational support (Ojo et al., 2022).
These weaknesses negatively affect the quality of health services delivered, impair planning and resource allocation, and weaken disease surveillance efforts (NPHCDA, 2019). PHCs often lack basic ICT equipment and internet access, leading to reliance on manual record-keeping, which increases the risk of errors and data loss. Furthermore, limited staff competence in HIS management undermines efforts to maintain accurate and timely patient and community health records (Awoonor-Williams et al., 2021).
Given these challenges, there is a pressing need to understand the specific barriers to HIS implementation in selected PHCs in Lokoja. Such knowledge is essential to develop practical interventions that improve data management, enhance service delivery, and ultimately strengthen primary health care in Nigeria (Olajide et al., 2020).
1.3 Research Objectives
General Objective:
To examine the challenges of implementing Health Information Systems in Primary Health Care Centres in Lokoja, Nigeria.
Specific Objectives:

To identify technical and infrastructural challenges hindering HIS implementation in selected PHCs in Lokoja.

1.5 Hypothesis
Ho: There is no significant relationship between health worker training and the effective implementation of Health Information Systems in Primary Health Care Centres in Lokoja.
1.6 Significance of the Study
This study will provide critical insights into the challenges of HIS implementation at the PHC level, which can guide policymakers, health administrators, and donors in designing targeted interventions. Understanding these barriers will support improvements in staff training, infrastructure development, and organizational practices, ultimately enhancing data-driven decision-making and service delivery (WHO, 2016; Ojo et al., 2022). Additionally, the study will contribute to academic literature on health information systems in low-resource settings and serve as a reference for future research on PHC strengthening in Nigeria (Awoonor-Williams et al., 2021).
1.7 Scope of the Study
The study focuses on selected PHCs in Lokoja, Kogi State, examining technical, human, and organizational challenges affecting HIS implementation. It covers HIS functionality, staff capacity, data management practices, and reporting processes but does not extend to secondary or tertiary health facilities.
