Introduction
1.1 Background of the Study
Child mortality remains a critical public health concern globally, particularly in developing countries. According to the World Health Organization (WHO, 2022), approximately 5.2 million children under five died in 2019, with preventable diseases being the major contributors. Sub-Saharan Africa bears the highest burden, accounting for nearly half of these deaths. In Nigeria, the under-five mortality rate stands at 104 deaths per 1,000 live births, reflecting inadequate healthcare services, poor access to immunization, and preventable infections (National Population Commission [NPC] & ICF, 2019).
Immunization is recognized as one of the most cost-effective public health interventions to reduce child morbidity and mortality. Vaccines protect children from deadly diseases such as measles, polio, tuberculosis, diphtheria, pertussis, and tetanus. Studies have shown that effective immunization programs can reduce child mortality by up to 25–30% in high-risk populations (WHO, 2021; Owais et al., 2019). Despite these benefits, the effectiveness of immunization programs in Nigeria varies widely due to factors such as poor healthcare infrastructure, low community awareness, vaccine hesitancy, and socio-economic barriers (Adebayo et al., 2020).
Anambra State, located in southeastern Nigeria, has made significant efforts to improve child health through routine immunization programs, including the Expanded Program on Immunization (EPI). However, discrepancies in immunization coverage exist among different Local Government Areas (LGAs), leading to variations in child mortality rates (NPHCDA, 2021). Evaluating the effectiveness of immunization programs in selected LGAs of Anambra State is therefore essential to understand the contribution of these programs to reducing child mortality and identifying gaps that may hinder their impact.
1.2 Statement of the Problem
Child mortality remains unacceptably high in Nigeria, despite national and international efforts to improve child health outcomes. In Anambra State, reports indicate that some LGAs still experience high under-five mortality rates, often linked to incomplete immunization coverage and the prevalence of vaccine-preventable diseases (NPHCDA, 2021).
While immunization programs are intended to prevent childhood diseases, challenges such as poor access to health facilities, inadequate vaccine supply, low community awareness, and cultural misconceptions limit their effectiveness (Adebayo et al., 2020). These challenges may result in partial immunization, delayed vaccination, or complete non-vaccination, contributing to preventable child deaths. Consequently, there is a need to evaluate how immunization programs are impacting child mortality rates in selected LGAs of Anambra State. This evaluation will provide evidence for policymakers, healthcare providers, and community stakeholders to improve the reach, quality, and effectiveness of immunization programs.
1.3 Research Objectives
The primary objective of this study is to evaluate the effectiveness of immunization programs on child mortality rates in selected Local Government Areas in Anambra State. The specific objectives are:
