CHAPTER ONE
INTRODUCTION
1.1 Background of the Study
Malaria remains one of the most important public health challenges in sub-Saharan Africa, with Nigeria bearing the highest burden globally. It is estimated that Nigeria contributes a significant proportion of global malaria cases and deaths annually, particularly among children under five years, pregnant women, and populations in rural and semi-urban areas (World Health Organization, 2023). Accurate and timely diagnosis of malaria is a critical component of malaria control strategies, as it ensures appropriate treatment, prevents overuse of antimalarial drugs, and reduces the development of drug resistance.
Traditionally, malaria diagnosis relied heavily on clinical symptoms such as fever, chills, and headache. However, these symptoms are non-specific and often overlap with other febrile illnesses, leading to frequent misdiagnosis and inappropriate treatment. To improve diagnostic accuracy, laboratory-based methods such as microscopy and Rapid Diagnostic Tests (RDTs) have been introduced as standard tools for malaria confirmation. Microscopy, which involves the examination of stained blood smears under a microscope, is considered the gold standard due to its ability to detect parasite species and quantify parasite density. However, its accuracy depends on the skill of laboratory personnel, quality of reagents, and availability of functional equipment (Olliaro et al., 2019).
Rapid Diagnostic Tests (RDTs), on the other hand, detect malaria antigens in human blood and provide results within 15–20 minutes without the need for advanced laboratory infrastructure. RDTs have significantly improved malaria diagnosis in primary health care settings, especially in resource-limited areas where microscopy services are unavailable or unreliable. Despite their advantages, RDTs have been reported to have variable sensitivity and specificity depending on parasite density, test quality, storage conditions, and operator competence (Okeke et al., 2020).
Studies conducted in Nigeria and other malaria-endemic regions have shown that while RDTs perform reasonably well in detecting Plasmodium falciparum, their accuracy may decline in cases of low parasitaemia or mixed infections. Conversely, microscopy, although more accurate when properly performed, is prone to errors due to poor staining techniques, inadequate training, and workload pressure in many health facilities (Ukpai & Ajoku, 2021).
In Nigeria, Primary Health Care (PHC) centres serve as the first point of contact for most patients seeking treatment for febrile illnesses, including malaria. These centres play a vital role in malaria case management and are often equipped with either microscopy, RDTs, or both. However, the diagnostic accuracy of malaria tests in these settings is influenced by multiple factors such as availability of trained personnel, adherence to standard operating procedures, quality control measures, and availability of functional diagnostic tools.
In Ogun State, malaria remains highly endemic due to favourable environmental conditions that support mosquito breeding, including stagnant water, high rainfall, and poor sanitation in some communities. Despite ongoing malaria control interventions, including distribution of insecticide-treated nets (ITNs) and intermittent preventive treatment, accurate diagnosis remains a challenge in many Primary Health Care centres. There is limited updated evidence on how well malaria diagnostic methods perform in real-world PHC settings in the state, particularly in terms of sensitivity, specificity, and overall diagnostic accuracy.
Therefore, evaluating the performance of malaria diagnostic methods in selected Primary Health Care centres in Ogun State is essential for improving case management, strengthening laboratory services, and supporting evidence-based malaria control strategies.
1.2 Statement of the Problem
Despite significant advancements in malaria diagnosis and control, malaria remains a leading cause of morbidity and mortality in Nigeria, largely due to challenges in accurate and timely diagnosis. In many Primary Health Care centres in Ogun State, malaria diagnosis is still heavily reliant on either Rapid Diagnostic Tests (RDTs) or microscopy, both of which have inherent limitations that may compromise diagnostic accuracy.
Misdiagnosis of malaria is a major concern in PHC settings. Over-reliance on clinical symptoms or poorly performed diagnostic tests often results in incorrect treatment, leading to either over-diagnosis or under-diagnosis of malaria cases. Over-diagnosis leads to unnecessary use of antimalarial drugs, contributing to drug resistance and wastage of resources, while under-diagnosis may result in untreated malaria cases, increasing the risk of severe complications and death.
Furthermore, the accuracy of malaria diagnostic methods in many PHC centres is affected by several operational challenges, including inadequate training of laboratory personnel, poor storage conditions of RDT kits, lack of quality control measures, and inconsistent adherence to standard diagnostic procedures. Microscopy, although considered the gold standard, is often compromised by poor slide preparation, staining errors, and lack of regular proficiency testing.
In Ogun State, there is limited empirical data comparing the performance of malaria diagnostic methods such as microscopy and RDTs in Primary Health Care settings. Most available studies are either hospital-based or outdated, and do not reflect current realities in PHC centres where the majority of malaria cases are first managed. This creates a significant gap in knowledge regarding the actual diagnostic accuracy of these methods in routine practice.
The absence of reliable and updated data on the performance of malaria diagnostic tools in PHC centres makes it difficult for health authorities to evaluate existing diagnostic systems, implement quality improvement strategies, and ensure adherence to national malaria diagnosis guidelines.
Therefore, this study seeks to address this gap by evaluating malaria diagnostic methods and their accuracy in selected Primary Health Care centres in Ogun State, Nigeria.
1.3 Objectives of the Study
The main objective of this study is to evaluate malaria diagnostic methods and their accuracy in selected Primary Health Care centres in Ogun State, Nigeria.
Specific objectives are to:
- Determine the prevalence of malaria using microscopy and Rapid Diagnostic Tests (RDTs).
- Compare the sensitivity and specificity of RDTs and microscopy in malaria diagnosis.
- Assess the level of agreement between microscopy and RDT results.
- Identify factors affecting the accuracy of malaria diagnostic methods in PHC centres.
1.4 Research Questions
- What is the prevalence of malaria using microscopy and RDTs in selected PHC centres?

1.5 Research Hypothesis
H₀: There is no significant difference in the diagnostic accuracy of Rapid Diagnostic Tests (RDTs) and microscopy in detecting malaria among patients in selected Primary Health Care centres in Ogun State.
H₁: There is a significant difference in the diagnostic accuracy of Rapid Diagnostic Tests (RDTs) and microscopy in detecting malaria among patients in selected Primary Health Care centres in Ogun State.
1.6 Significance of the Study
This study will be beneficial to healthcare professionals, laboratory scientists, and public health policymakers. It will provide evidence-based information on the performance of malaria diagnostic methods in PHC settings, which can be used to improve diagnostic accuracy and patient management. The findings will also guide training programs for laboratory personnel and support the strengthening of quality assurance systems in malaria diagnosis. Additionally, it will contribute to academic literature and serve as a reference for future research in malaria diagnostics.
1.7 Scope of the Study
This study is limited to selected Primary Health Care centres in Ogun State, Nigeria. It focuses on the evaluation of malaria diagnostic methods, specifically microscopy and Rapid Diagnostic Tests (RDTs), and assesses their accuracy in terms of sensitivity, specificity, and agreement between both methods.
1.8 Operational Definition of Terms
- Malaria: A parasitic disease caused by Plasmodium species transmitted through the bite of infected Anopheles mosquitoes.
- Microscopy: A laboratory method of malaria diagnosis involving examination of stained blood smears under a microscope.
- Rapid Diagnostic Test (RDT): A quick test that detects malaria antigens in blood without the need for microscopy.
- Sensitivity: The ability of a test to correctly identify individuals with malaria.
- Specificity: The ability of a test to correctly identify individuals without malaria.
- Primary Health Care Centre: A first-level healthcare facility providing basic medical services to the community.
