INTRODUCTION
1.1 Background to the Study
Disease surveillance remains a fundamental component of public health systems because it provides the information required for detecting, monitoring, preventing, and responding to diseases. Effective surveillance systems enable health authorities to identify disease patterns, recognize outbreaks early, allocate resources appropriately, and implement timely interventions. Traditionally, disease surveillance in many low- and middle-income countries has depended on paper-based reporting systems, manual data aggregation, and physical transmission of health records from primary healthcare facilities to higher administrative levels. Although these approaches have contributed significantly to public health monitoring, they are often associated with delays, incomplete reporting, data inaccuracies, and limited capacity for real-time decision-making (World Health Organization [WHO], 2021).
The rapid advancement of digital technologies has introduced new possibilities for improving healthcare delivery and strengthening disease surveillance systems. Digital health technologies refer to the use of information and communication technologies (ICTs), including electronic health records, mobile health applications, digital reporting platforms, electronic disease surveillance systems, telemedicine, and data analytics tools, to support healthcare services and public health activities. These technologies have increasingly become important tools for improving data collection, storage, analysis, and communication within healthcare systems (WHO, 2021).
Globally, digital health has emerged as a major strategy for improving health system performance. Many countries have adopted electronic surveillance platforms that allow healthcare workers to report disease cases electronically, monitor trends, and communicate public health information rapidly. During infectious disease outbreaks such as Ebola and COVID-19, digital technologies demonstrated their importance in enhancing real-time surveillance, contact tracing, and emergency response coordination (Mbunge et al., 2021). These experiences highlighted the potential of digital health solutions to overcome limitations associated with conventional surveillance systems.
In developing countries, however, the adoption of digital health technologies remains uneven due to challenges related to infrastructure, digital literacy, funding limitations, and inadequate policy implementation. Many healthcare systems in low-resource settings continue to struggle with weak surveillance mechanisms, poor internet connectivity, unreliable electricity supply, and insufficient training of healthcare personnel. These challenges affect the ability of health workers to utilize digital tools effectively and reduce the potential benefits of technology-driven surveillance systems (Agarwal et al., 2021).
Nigeria, as Africa’s most populous country, faces significant public health challenges requiring effective disease surveillance and reporting systems. The country experiences recurrent outbreaks of infectious diseases, including malaria, tuberculosis, cholera, Lassa fever, meningitis, and emerging diseases. Effective surveillance is therefore critical for identifying health threats and coordinating appropriate responses. The Nigeria Centre for Disease Control (NCDC) has strengthened disease surveillance through initiatives such as the Surveillance Outbreak Response Management and Analysis System (SORMAS), an electronic surveillance platform designed to improve reporting, analysis, and outbreak response capacity (Adeoye et al., 2021).
Primary healthcare workers occupy a strategic position within Nigeria’s disease surveillance structure because they serve as the first point of contact between communities and the formal healthcare system. Primary healthcare facilities are responsible for identifying suspected cases, collecting health information, reporting priority diseases, and participating in outbreak response activities. The effectiveness of national surveillance systems therefore depends significantly on the ability of primary healthcare workers to accurately collect and report disease information.
Despite the importance of primary healthcare workers, disease surveillance and reporting at the primary healthcare level in Nigeria face persistent challenges. Many facilities continue to experience delays in submitting reports, incomplete documentation, poor data quality, and limited feedback from higher administrative levels. These challenges reduce the usefulness of surveillance information for timely public health decision-making. According to Ojo et al. (2022), weaknesses in health information management systems and limited technical capacity among healthcare workers remain major barriers affecting healthcare data utilization in Nigeria.
Digital health technologies have the potential to address several weaknesses associated with traditional surveillance approaches. Electronic reporting systems can reduce reporting delays by allowing healthcare workers to submit disease information directly through digital platforms. Automated data validation features can improve accuracy by reducing errors associated with manual entry. Furthermore, digital platforms can enhance communication between healthcare facilities and public health authorities by enabling faster exchange of information.
The use of digital health technologies can also improve disease surveillance effectiveness by strengthening data accessibility and analysis. Real-time digital data allows health authorities to monitor disease trends, identify unusual increases in cases, and initiate preventive measures before outbreaks escalate. In addition, digital platforms can provide healthcare workers with access to updated surveillance guidelines, decision-support tools, and training resources, thereby improving their capacity to perform surveillance responsibilities effectively (Labrique et al., 2018).
The COVID-19 pandemic further demonstrated the importance of digital health technologies in strengthening health information systems. During the pandemic, digital tools were used globally for case reporting, laboratory data management, risk communication, and monitoring public health interventions. In Nigeria, digital platforms supported disease reporting and outbreak management by improving coordination between healthcare facilities and surveillance authorities (Adebisi et al., 2021). The experience reinforced the need for sustainable investment in digital health infrastructure.
However, the effectiveness of digital health technologies depends not only on the availability of technology but also on the readiness and capacity of healthcare workers who use them. Primary healthcare workers require adequate training, technical support, and organizational encouragement to effectively integrate digital tools into routine surveillance activities. Without sufficient knowledge and confidence, digital systems may remain underutilized or fail to produce expected improvements in reporting effectiveness.
Furthermore, differences in healthcare facility characteristics may influence the success of digital health adoption. Urban healthcare facilities may have better access to internet connectivity, electricity, and trained personnel compared with rural facilities. These disparities may affect the extent to which digital health technologies improve surveillance outcomes across different regions of Nigeria. Therefore, understanding the experiences and perceptions of primary healthcare workers is necessary for developing strategies that promote effective digital health implementation.
Although Nigeria has made progress in adopting digital health solutions, empirical evidence regarding their influence on disease surveillance and reporting effectiveness among primary healthcare workers remains limited. Existing studies have examined digital health adoption, electronic reporting systems, and health information management separately, but fewer studies have specifically investigated how digital technologies influence surveillance performance among frontline healthcare workers.
This study is therefore important because it seeks to examine the influence of digital health technologies on disease surveillance and reporting effectiveness among primary healthcare workers in Nigeria. Understanding this relationship will provide evidence for improving digital health policies, strengthening primary healthcare systems, and enhancing Nigeria’s capacity to detect and respond to public health threats.
1.2 Statement of the Problem
Disease surveillance and reporting remain major challenges within Nigeria’s public healthcare system despite continuous efforts by government agencies and international partners to strengthen health information management. An effective surveillance system requires timely, accurate, and complete reporting of disease information from healthcare facilities, especially primary healthcare centres where many communities first interact with the healthcare system. However, weaknesses in reporting practices continue to limit the ability of health authorities to obtain reliable information for disease prevention and control.
One major problem affecting disease surveillance in Nigeria is the continued reliance on manual and fragmented reporting processes in many primary healthcare facilities. Paper-based documentation and traditional reporting procedures often result in delays between disease identification and submission of surveillance reports. Such delays may prevent health authorities from responding quickly to outbreaks, thereby increasing the risk of disease transmission and poor health outcomes.
Another problem is the issue of data quality within disease surveillance systems. Inaccurate, incomplete, or inconsistent reporting can affect the reliability of health information used for decision-making. Primary healthcare workers may encounter difficulties in recording, compiling, and submitting surveillance data due to workload pressures, limited training, and inadequate supervision. These challenges reduce the effectiveness of disease monitoring activities.
The introduction of digital health technologies has been viewed as a potential solution to these challenges. Electronic reporting platforms, mobile health applications, and digital surveillance systems are expected to improve the speed, accuracy, and accessibility of disease information. However, the actual impact of these technologies among primary healthcare workers in Nigeria remains unclear.
Although platforms such as SORMAS have improved disease surveillance capacity in some parts of Nigeria, challenges relating to adoption, sustainability, infrastructure, and user competency remain. Some primary healthcare workers experience difficulties using digital platforms due to inadequate training, limited technical support, poor internet access, and equipment challenges. Consequently, the availability of digital technologies does not automatically guarantee improved surveillance outcomes.
Furthermore, there is insufficient empirical evidence explaining whether digital health technologies significantly improve disease surveillance and reporting effectiveness among primary healthcare workers. While previous studies have emphasized the importance of digital transformation in healthcare, limited attention has been given to the direct relationship between digital technology utilization and surveillance performance at the primary healthcare level.
The lack of adequate evidence creates uncertainty for policymakers and healthcare administrators regarding investments in digital health systems. Without understanding the extent to which digital technologies improve reporting effectiveness, resources may not be directed toward interventions that produce the greatest improvement in surveillance outcomes.
Therefore, this study addresses the problem of inadequate understanding of the influence of digital health technologies on disease surveillance and reporting effectiveness among primary healthcare workers in Nigeria. The study seeks to provide empirical evidence on whether digital health adoption contributes significantly to improved disease reporting practices and surveillance performance.
1.3 Aim of the Study
The main aim of this study is to examine the influence of digital health technologies on disease surveillance and reporting effectiveness among primary healthcare workers in Nigeria.
1.4 Objectives of the Study
The specific objectives are to:
- Examine the level of adoption of digital health technologies among primary healthcare workers in Nigeria.

1.6 Research Hypothesis
H₀: Digital health technologies have no significant influence on disease surveillance and reporting effectiveness among primary healthcare workers in Nigeria.
1.7 Significance of the Study
The findings of this study will be significant to healthcare policymakers, particularly government agencies responsible for strengthening Nigeria’s health information systems. The findings may provide evidence for improving digital health policies and investments aimed at enhancing disease surveillance.
Primary healthcare administrators and managers may benefit from the study by understanding factors affecting the successful implementation of digital surveillance tools. The findings may guide training programmes, supervision strategies, and resource allocation.
Primary healthcare workers may benefit from improved understanding of how digital technologies can support their surveillance responsibilities. The study may encourage greater acceptance and effective utilization of digital reporting systems.
The study will also contribute to academic knowledge by providing empirical evidence on digital health adoption and public health surveillance in Nigeria. Researchers may use the findings as a reference for further studies in digital health and health information management.
1.8 Scope of the Study
