INTRODUCTION
1.1 Background of the Study
Coronavirus Disease 2019 (COVID-19), caused by the Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2), was first reported in Wuhan, China in late 2019 and rapidly escalated into a global pandemic, prompting the World Health Organization (WHO) to declare it a Public Health Emergency of International Concern in January 2020. The disease spread to almost every country in the world, with Nigeria reporting its first case on February 27, 2020, and subsequent widespread transmission across urban and rural areas. The outbreak has placed significant strain on public health systems, economies, and community wellbeing globally.
Effective control of COVID-19 has hinged not only on biomedical interventions such as vaccines and treatments but also on public adherence to preventive measures including hand hygiene, mask-wearing, social distancing, and compliance with public health advisories. The success of these measures largely depends on the knowledge, attitude, and practice (KAP) of individuals and communities toward the disease and its prevention strategies. High levels of knowledge about the disease and positive attitudes toward prevention have been shown to correlate with better engagement in protective practices, thus reducing transmission risks.
Numerous KAP studies worldwide have demonstrated that adequate knowledge of COVID-19, a positive attitude toward preventive measures, and correct practices such as frequent handwashing and proper mask use are critical in mitigating the spread of SARS-CoV-2. However, gaps still exist in translating knowledge and attitude into consistent preventive practices, especially in resource-limited settings. In sub-Saharan Africa, including Nigeria, variations in socio-demographic characteristics, access to reliable health information, cultural beliefs, education levels, and economic conditions significantly influence how communities perceive, understand, and respond to public health guidance.
Within Nigeria, studies conducted in various states have highlighted diverse outcomes in KAP levels among different population groups. For example, an investigation among clients attending primary healthcare facilities in Rivers State suggested high knowledge and positive attitudes towards COVID-19 among respondents, but adherence to preventive practices varied by age, educational status, and location. Such findings underscore the value of localized research because perceptions and behaviours are shaped by context-specific factors, including local health communication strategies, trust in government directives, and socio-economic realities.
Obio-Akpor Local Government Area (LGA), situated in Rivers State, is an urbanized region with dense populations, mixed socio-economic profiles, and diverse cultural backgrounds. As a key urban centre, Obio-Akpor faced substantial COVID-19 risk due to high population mobility and frequent interpersonal contact in market places, transportation hubs, schools, and workplaces. Yet, despite national and state-level public health campaigns during the pandemic, there remains limited empirical evidence on how residents of Obio-Akpor specifically understood COVID-19, what attitudes they held toward preventive measures, and the extent to which knowledge and attitudes translated into protective practices.
Understanding the KAP toward COVID-19 prevention in Obio-Akpor is important for several reasons. First, localized insights can help health authorities tailor communication and intervention strategies that resonate with residents’ beliefs, information sources, and perceived barriers. Second, by identifying gaps between knowledge and practice, policymakers can design targeted community engagement efforts to strengthen uptake of recommended behaviours. Finally, examining socio-demographic correlates of KAP will support equitable public health planning that addresses disparities among different population groups within the LGA.
1.2 Statement of the Problem
Although widespread public health campaigns were implemented across Nigeria to promote COVID-19 preventive behaviours, varying levels of knowledge, attitudes, and practices continue to be reported. In Rivers State, existing research indicates high overall awareness of COVID-19 among healthcare facility clients, but adherence to preventive practices remains influenced by socio-demographic factors such as educational status, occupation, and senatorial districts.
In Obio-Akpor LGA, there is currently insufficient localized evidence quantifying community members’ knowledge of COVID-19’s transmission and prevention, their attitudes toward public health measures, and the extent to which they consistently engage in protective practices. Without such evidence, public health authorities are constrained in their ability to identify gaps in awareness and behavioural compliance, or to determine whether ongoing communication strategies have effectively translated into real-world prevention behaviours.
Moreover, the dynamics of urban living—such as high population density, social gatherings, and economic pressures—can shape behavioural responses to infectious disease outbreaks differently from rural areas. Misconceptions, misinformation, and varying levels of trust in government messages may also influence attitudes and practices at the community level. Despite the relevance of these factors, few studies have focused specifically on Obio-Akpor residents, leaving a critical knowledge gap in understanding the KAP landscape in this urban Nigerian context.
This study therefore seeks to fill this gap by assessing the levels of knowledge, attitudes, and practices toward COVID-19 prevention among residents of Obio-Akpor LGA. The findings will provide evidence to inform contextualized public health strategies, address barriers to preventive behaviour adoption, and contribute to broader understanding of community responses to pandemic threats in urban Nigerian settings.
1.3 Objectives of the Study

To assess the knowledge, attitude, and practice toward COVID-19 prevention among residents of Obio-Akpor Local Government Area, Rivers State.

1.5 Research Hypothesis
H₁: There is a significant positive relationship between the level of knowledge about COVID-19 and the adoption of preventive practices among residents of Obio-Akpor Local Government Area, Rivers State.
1.6 Significance of the Study
The study will provide empirical evidence on the community’s knowledge, attitudes, and practices toward COVID-19 prevention in Obio-Akpor LGA. The findings will help public health officials, Lagos State Ministry of Health counterparts, and local policymakers to design targeted education campaigns, improve risk communication strategies, and address behavioural barriers to effective disease prevention. Additionally, the study adds to the academic literature on COVID-19 KAP in urban Nigerian contexts and can serve as a reference for future epidemic preparedness research.
1.7 Scope of the Study
This study focuses on adult residents of Obio-Akpor LGA, Rivers State. It examines their knowledge of COVID-19, attitudes toward preventive measures, and actual practices implemented to reduce infection risk. The study does not include clinical measurements of COVID-19 infection status or assess vaccine efficacy.
