CHAPTER ONE

INTRODUCTION

1.1 Background of the Study

Human Immunodeficiency Virus (HIV) and Acquired Immunodeficiency Syndrome (AIDS) continue to be a significant global public health challenge, particularly among adolescents in sub-Saharan Africa (UNAIDS, 2022). Adolescents are at increased risk due to their stage of psychosocial development, experimentation with sexual activity, peer influence, and limited access to comprehensive sexual and reproductive health information (UNICEF, 2021). Globally, an estimated 1.7 million adolescents aged 10–19 years are living with HIV, and the majority of new infections occur in sub-Saharan Africa (WHO, 2023).

In Nigeria, HIV prevalence among adolescents remains concerning. The Nigeria National Agency for the Control of AIDS (NACA, 2022) reports that adolescents account for a significant proportion of new HIV infections annually, reflecting gaps in knowledge, inadequate preventive measures, and social and cultural barriers. HIV/AIDS prevention strategies include abstinence, delayed sexual debut, consistent condom use, voluntary counseling and testing (VCT), and awareness campaigns (Avert, 2022). The success of these strategies depends largely on adolescents’ knowledge, attitudes, and practices (KAP) regarding the disease.

Nsukka, in Enugu State, is home to a mix of urban and semi-urban populations, including a large youth population due to the presence of the University of Nigeria and surrounding educational institutions. Despite awareness campaigns and school-based programs, adolescents in Nsukka may still be exposed to high-risk behaviors due to peer pressure, lack of parental guidance, poor access to youth-friendly services, and misconceptions about HIV/AIDS transmission (Okeke et al., 2020). Additionally, cultural beliefs and stigma surrounding HIV may prevent adolescents from seeking testing or adopting preventive measures.

Understanding adolescents’ knowledge, attitudes, and practices is crucial for designing effective, localized interventions. Such assessments provide insights into misconceptions, behavioral gaps, and social or environmental factors that influence HIV risk. By evaluating KAP in Nsukka, stakeholders—including health educators, policymakers, and NGOs—can design targeted interventions that address the specific needs of adolescents and reduce HIV prevalence in the community.

1.2 Statement of the Problem

HIV/AIDS continues to pose a significant threat to adolescent health in Nigeria. In Nsukka, studies indicate that despite national awareness campaigns, adolescents remain vulnerable to HIV infection due to low levels of comprehensive knowledge, risky sexual behavior, and inconsistent adoption of preventive measures (Okeke et al., 2020).

Key challenges include socio-cultural barriers that limit open discussions about sexual health, economic factors that may drive adolescents to engage in transactional sex, limited access to youth-friendly health services, and pervasive stigma surrounding HIV/AIDS (Eze & Nwankwo, 2021). These factors compromise preventive practices, increasing the likelihood of new infections among adolescents.

Currently, there is limited empirical evidence specifically assessing the knowledge, attitudes, and practices regarding HIV/AIDS prevention among adolescents in Nsukka. Without such data, interventions may be generic and fail to target the specific gaps in adolescent behavior and awareness. This study seeks to fill this gap by providing a comprehensive assessment of KAP concerning HIV/AIDS prevention among adolescents in Nsukka, Enugu State, to guide effective public health strategies.

1.3 Research Objectives

The general objective of this study is to assess the knowledge, attitude, and practice of HIV/AIDS prevention among adolescents in Nsukka, Enugu State. The specific objectives are to:

1. Determine the level of knowledge of HIV/AIDS transmission and prevention among adolescents in Nsukka.

2. Assess the attitudes of adolescents toward HIV/AIDS preventive measures.

3. Examine the preventive practices adopted by adolescents regarding HIV/AIDS.

4. Identify socio-demographic and environmental factors influencing HIV/AIDS preventive behaviors among adolescents.

1.4 Research Questions

1. What is the level of knowledge of HIV/AIDS transmission and prevention among adolescents in Nsukka?

2. What are the attitudes of adolescents toward HIV/AIDS preventive measures in the study area?

3. What are the current practices of adolescents regarding HIV/AIDS prevention?

4. Which socio-demographic and environmental factors influence adolescents’ HIV/AIDS preventive behaviors?

1.5 Research Hypothesis

H₀: There is no significant relationship between adolescents’ socio-demographic factors and their knowledge, attitude, and practice of HIV/AIDS prevention in Nsukka, Enugu State.
H₁: There is a significant relationship between adolescents’ socio-demographic factors and their knowledge, attitude, and practice of HIV/AIDS prevention in Nsukka, Enugu State.

1.6 Significance of the Study

This study will provide empirical data on adolescents’ knowledge, attitudes, and practices regarding HIV/AIDS prevention in Nsukka, which will guide policymakers, health educators, and NGOs in designing evidence-based interventions. Additionally, the study will contribute to the academic literature on adolescent sexual health in Nigeria and provide insights into behavioral and socio-cultural factors that influence HIV prevention among youth.

1.7 Scope of the Study

The study focuses on adolescents aged 10–19 years residing in Nsukka, Enugu State. It examines their knowledge, attitudes, and practices regarding HIV/AIDS prevention, as well as socio-demographic and environmental factors influencing preventive behaviors. While the study is limited to Nsukka, the findings may have implications for similar adolescent populations in other parts of Enugu State and Nigeria.

1.8 Definition of Terms

1. HIV/AIDS: Human Immunodeficiency Virus (HIV) is a virus that attacks the immune system, and Acquired Immunodeficiency Syndrome (AIDS) is the advanced stage of HIV infection (WHO, 2023).

2. Adolescents: Individuals aged 10–19 years, undergoing the transition from childhood to adulthood.

3. Knowledge: Awareness and understanding of HIV/AIDS transmission, prevention methods, and consequences of infection.

4. Attitude: Adolescents’ perceptions, beliefs, and feelings toward HIV/AIDS prevention measures.

5. Practice: The actual behaviors and actions undertaken by adolescents to prevent HIV/AIDS infection, such as condom use, abstinence, or HIV testing.

6. Prevention: Measures taken to reduce the risk of contracting HIV, including safe sexual practices, education, and voluntary testing.
