INTRODUCTION
1.1 Background of the Study
Health Information Management (HIM) encompasses the systematic collection, storage, analysis, retrieval, and dissemination of health data used for planning, monitoring, evaluation, and decision‑making within health systems (WHO, 2025). A key component of HIM is disease surveillance and reporting, which involves gathering timely and accurate information on infectious diseases for early detection, effective public health response, and control of outbreaks (Isere et al., 2015; Nnebue et al., 2012). Within the Nigerian context, disease surveillance is implemented through the Integrated Disease Surveillance and Response (IDSR) strategy — a framework adopted to strengthen national capacities for detecting and responding to epidemic‑prone diseases (Fasuan et al., 2025). Disease surveillance and reporting systems serve as “early warning” mechanisms, enabling health authorities to monitor trends, detect outbreaks, and institute appropriate responses in a timely manner (Isere et al., 2015; WHO, 2025).
Infectious diseases such as malaria, tuberculosis, cholera, measles, and more recently COVID‑19, remain significant public health challenges in Nigeria, causing high morbidity and mortality and placing pressure on health systems (Critical Analysis of Infectious Disease Surveillance, 2025). Effective surveillance relies heavily on accurate and timely health data gathered from health facilities, particularly specialized infectious disease clinics where reporting can signal emerging public health threats. In this regard, the role of HIM is crucial because it ensures that disease data are systematically processed and used for action. High‑quality health information contributes to evidence‑based decisions that support outbreak prevention, resource allocation, and evaluation of intervention effectiveness (WHO, 2025).

1.2 Statement of the Problem
Although disease surveillance and reporting are central to public health planning and response, evidence suggests that health information management practices in many Nigerian health facilities — especially at the level of infectious disease clinics — are fraught with persistent challenges. Weak data management systems have been shown to contribute to delays in detecting and reporting infectious disease cases, which in turn hinder outbreak responses and effective control measures (Critical Analysis of Infectious Disease Surveillance, 2025). In Enugu State, evaluations of the Integrated Disease Surveillance and Response system — which forms the basis of the national surveillance architecture — reveal performance indicators significantly below recommended standards, highlighting issues such as poor documentation and suboptimal reporting practices (Evaluation of IDSR in Enugu State, 2018).
Further compounding the problem, studies indicate that many health workers, while aware of disease surveillance systems, lack adequate knowledge of surveillance tools, notification requirements, and proper reporting procedures (Nnebue et al., 2012). Given these challenges, there is a clear need to examine specifically how health information management influences disease surveillance and reporting within infectious disease clinics at the University of Nigeria Teaching Hospital (UNTH), Enugu. UNTH operates specialized clinics where surveillance data should ideally be managed with high precision, yet the extent to which HIM supports timely, accurate reporting in this setting is not well documented. Without such investigation, policymakers and health administrators lack the evidence necessary to strengthen HIS frameworks, improve health worker training, and enhance the quality of public health data — all critical for effective infectious disease control.
1.3 Research Objectives

1.5 Hypothesis

Findings will be valuable to hospital administrators, public health policymakers, and health information managers seeking to strengthen surveillance systems and improve outbreak detection and response. By pinpointing specific barriers and facilitators of effective reporting, the study can inform targeted interventions such as training programs, HIS infrastructure improvements, and surveillance protocol enhancements. Additionally, the research will contribute to academic knowledge on HIS and public health surveillance in low‑resource settings, serving as a reference for future studies and policy discussions in Nigeria and similar contexts.
1.7 Scope of the Study
This research focuses on infectious disease clinics within the University of Nigeria Teaching Hospital (UNTH), Enugu. It examines how health information management practices affect disease surveillance and reporting, including aspects of data collection, health worker knowledge, barriers to reporting, and quality of surveillance data. The study does not extend to general outpatient departments or other hospitals outside UNTH.
