INTRODUCTION

• Background to the Study

The relationship between abortion and mortality among young women has been the subject of increasing academic scrutiny. Several studies have highlighted that while abortion is generally a safe medical procedure when performed legally and under clinical conditions, disparities in access and post-abortion care significantly impact outcomes. According to the World Health Organization (2021), unsafe abortions are a major contributor to maternal mortality, especially in low- and middle-income countries, where access to safe abortion services is often restricted. Young women, often facing barriers such as stigma, lack of information, and economic dependence, are more likely to seek clandestine procedures, heightening their risk of mortality.

A number of retrospective cohort studies from high-income countries provide contrasting insights. For instance, research by Gissler et al. (2005) in Finland found that women who had abortions had higher rates of death from all causes compared to those who gave birth, with a particularly elevated risk among younger women. However, the study acknowledged that confounding factors, such as underlying mental health issues or socioeconomic status, might partly explain this association. Similarly, Reardon et al. (2002) examined U.S. Medicaid data and found a correlation between abortion and increased mortality, primarily due to suicide, drug use, and accidents. This raised questions about the long-term psychosocial impact of abortion, especially among vulnerable youth.

1.2. Statement of the Problem

Despite advances in reproductive health and the legalization of abortion in many countries, abortion-related complications and mortality remain a critical issue, particularly among young women. Globally, unsafe abortions are a leading cause of maternal death, disproportionately affecting women under the age of 25. This raises an urgent public health concern regarding the systemic, legal, and social factors that make young women especially vulnerable to adverse outcomes related to abortion procedures.

In many parts of the world, restrictive abortion laws, combined with limited access to reproductive healthcare, drive young women toward unsafe and clandestine abortion methods. These procedures often occur in non-sterile environments, performed by unqualified individuals, and without post-abortion care. Young women, often lacking financial independence, education, or social support, are more likely to resort to these unsafe options. As a result, they are at greater risk of hemorrhage, infection, sepsis, and death—conditions that are largely preventable with access to safe, legal abortion services.

Furthermore, even in countries where abortion is legal, young women may still face significant barriers to safe abortion services, including parental consent laws, stigma, misinformation, and logistical obstacles such as transportation or cost. These barriers often delay the procedure, increasing health risks. Emotional distress, lack of adequate counseling, and societal pressure can further complicate the physical and mental health outcomes following an abortion. Without comprehensive post-abortion care, young women may suffer from untreated complications or psychological issues, which can contribute to indirect forms of mortality such as suicide or substance abuse.

1.6. Significance of the Study

This study is significant because it addresses a pressing public health concern in Omoku Metropolis, where abortion-related complications among young women may contribute to the rising rates of maternal morbidity and mortality. By examining abortion as a potential correlate to increased mortality, the study aims to generate critical insights into the specific risks faced by young women in this locality. Understanding the extent and nature of these risks can inform better health strategies, advocacy, and policy formulation tailored to the needs of the population in Omoku.

In a socio-cultural environment like Omoku, where traditional values, religious beliefs, and legal restrictions strongly influence reproductive choices, young women are often pushed toward unsafe abortion practices. These practices, conducted in secrecy and without adequate medical supervision, increase the likelihood of severe health outcomes, including death. This study seeks to shed light on how these socio-legal dynamics contribute to a silent but urgent health crisis. Highlighting these local realities is essential for creating interventions that are not only effective but also culturally sensitive and community-driven.

Furthermore, the study holds significance for healthcare providers and policymakers in Rivers State and beyond. By identifying gaps in reproductive health services and assessing the systemic barriers that prevent young women from accessing safe abortion and post-abortion care, this research can guide the allocation of healthcare resources. It can also support the training of healthcare personnel to be more responsive to the reproductive health needs of young women, especially in underserved areas like Omoku.

This research will also be valuable for educators, non-governmental organizations, and youth advocates working on reproductive health education. Findings from the study can be used to develop targeted awareness campaigns and educational programs aimed at reducing unsafe abortion practices among adolescents and young adults. These programs can play a key role in equipping young women with knowledge about contraception, their reproductive rights, and safe health-seeking behaviors, ultimately helping to reduce preventable deaths.

By filling this gap, the study provides a more nuanced understanding of the risks and lived experiences associated with abortion in the region.

Finally, the study’s findings could prompt community-based discussions and policy reforms that promote the rights and well-being of young women. If effectively disseminated, the results may help reduce the stigma around reproductive health conversations and push for youth-friendly health services in Omoku. This, in turn, can foster a more supportive environment where young women can make informed reproductive choices without fear, stigma, or risk to their lives.

1.7. Scope of the Study

The study examines abortion as a correlate or increase mortality among young women in Omoku Metropolis. The study is limited to the women young women in Omoku Metropolis, Rivers State.

1.8. Operational Definition of Terms

1. Abortion: Abortion is the medical or surgical termination of a pregnancy before the fetus can survive outside the womb. It may be performed intentionally (induced abortion) or occur naturally due to complications (spontaneous abortion or miscarriage).
2. Increase: Increase refers to a rise or growth in the number, amount, intensity, or level of something over time. In research or health studies, it often indicates a measurable upward trend in a specific variable.
3. Mortality: Mortality refers to the incidence or rate of death within a population during a specific period. It is commonly used in public health to assess the frequency of deaths due to particular causes, such as maternal mortality or disease-specific mortality.
4. Young Women: Young women generally refer to females typically between the ages of 15 and 24. This age group is often studied separately in health and social research due to their unique biological, psychological, and socio-economic challenges, especially regarding reproductive health.
