CHAPTER ONE
INTRODUCTION
1.1 Background of the Study
Malaria remains one of the most significant public health challenges in sub-Saharan Africa, accounting for substantial morbidity and mortality, particularly among vulnerable populations such as pregnant women and children under five years of age (WHO, 2023). Globally, the World Health Organization estimates over 200 million malaria cases annually, with Africa bearing more than 90% of these cases (WHO, 2023). In Nigeria, malaria is endemic, with pregnant women being highly susceptible due to changes in immunity during pregnancy, which increases their risk of severe disease, maternal anemia, and adverse pregnancy outcomes such as low birth weight and stillbirth (Nduka et al., 2020).
Malaria prevention strategies recommended by health authorities include the use of insecticide-treated nets (ITNs), intermittent preventive treatment in pregnancy (IPTp) with sulfadoxine-pyrimethamine, prompt diagnosis, and effective case management (National Malaria Elimination Programme [NMEP], 2022). Despite these measures, the prevalence of malaria among pregnant women remains high in many regions of Nigeria, including Kaduna State, due to factors such as poor knowledge of preventive measures, socio-economic barriers, cultural beliefs, and inconsistent utilization of healthcare services (Mohammed et al., 2021).
Zaria Local Government Area (LGA), Kaduna State, is an urban-rural mixed community where malaria transmission is perennial due to climatic conditions favorable to Anopheles mosquito breeding, poor environmental sanitation, and stagnant water sources (Ahmad et al., 2020). Although government programs and health facilities offer preventive interventions, the burden of malaria in pregnant women persists. Understanding both the prevalence of malaria and the factors that influence preventive behaviors in this population is essential for designing effective public health interventions.
Research shows that socio-demographic factors such as age, educational level, income, and parity, along with environmental and behavioral factors like housing conditions, use of ITNs, and attendance at antenatal care (ANC) services, significantly influence malaria prevention practices among pregnant women (Adebayo et al., 2019). These findings highlight the need for localized studies to identify barriers to prevention and optimize strategies for reducing malaria incidence among pregnant women in high-risk areas like Zaria LGA.
1.2 Statement of the Problem
Malaria in pregnancy remains a leading cause of maternal morbidity and mortality in Nigeria. Pregnant women in Zaria LGA continue to experience high rates of malaria infection despite the availability of preventive interventions such as ITNs and IPTp (NMEP, 2022). Non-compliance with preventive measures, inadequate awareness of malaria risks, and socio-economic constraints contribute to the persistence of the problem (Mohammed et al., 2021).
Moreover, environmental factors such as poor drainage, stagnant water, and substandard housing conditions facilitate mosquito breeding and increase exposure to malaria vectors. Inconsistent attendance at ANC clinics further limits access to preventive treatment and health education, thereby exacerbating malaria prevalence among pregnant women in the LGA (Ahmad et al., 2020).
Although several studies have assessed malaria prevalence and preventive practices in Nigeria, there is limited empirical data specific to Zaria LGA that simultaneously evaluates prevalence and identifies socio-demographic, behavioral, and environmental factors influencing malaria prevention among pregnant women. Addressing this gap is critical for evidence-based planning and targeted interventions that can reduce maternal morbidity and mortality linked to malaria in the area.
1.3 Research Objectives
The general objective of this study is to assess the prevalence of malaria and identify factors influencing its prevention among pregnant women in Zaria LGA, Kaduna State. The specific objectives are to:

1.5 Research Hypothesis
H₀: There is no significant relationship between socio-demographic, behavioral, and environmental factors and malaria prevention practices among pregnant women in Zaria LGA.
H₁: There is a significant relationship between socio-demographic, behavioral, and environmental factors and malaria prevention practices among pregnant women in Zaria LGA.
1.6 Significance of the Study
This study will provide empirical data on malaria prevalence and prevention practices among pregnant women in Zaria LGA, which can guide local health authorities, policymakers, and NGOs in designing targeted interventions. It will also contribute to academic knowledge on maternal health and malaria prevention in urban-rural communities in Nigeria. Moreover, findings from this research could support strategies to increase awareness, improve preventive behavior, and reduce malaria-related complications among pregnant women.
1.7 Scope of the Study
The study will focus on pregnant women attending antenatal clinics in Zaria LGA, Kaduna State. It will assess both malaria prevalence and factors influencing preventive practices, including socio-demographic, behavioral, and environmental aspects. The study is limited to this LGA, though findings may be useful for similar urban-rural communities in Nigeria.
1.8 Definition of Terms
