INTRODUCTION
1.1 Background of the Study
Prediabetes is a metabolic condition characterized by blood glucose levels that are higher than normal but below the threshold for a diagnosis of type 2 diabetes mellitus (T2DM) (American Diabetes Association [ADA], 2023). Individuals with prediabetes are at high risk of progressing to T2DM, cardiovascular diseases, and other metabolic complications (Tabák et al., 2012). Globally, over 374 million adults live with prediabetes, and without appropriate interventions, approximately 5–10% progress to diabetes annually (International Diabetes Federation [IDF], 2022).
Lifestyle factors, including unhealthy diet, physical inactivity, obesity, and stress, are major contributors to the development of prediabetes and its progression to T2DM (Tuomilehto et al., 2001). Community-based lifestyle interventions, which focus on promoting healthy eating, regular physical activity, and behavior modification, have been shown to be effective in reducing blood glucose levels, delaying or preventing the onset of T2DM, and improving overall metabolic health (Knowler et al., 2002; Li et al., 2008).
In Nigeria, prediabetes is increasingly recognized as a public health challenge, particularly in urban centers such as Ibadan, where rapid urbanization and lifestyle transitions have resulted in rising rates of obesity, poor dietary practices, and sedentary behavior (Adegoke et al., 2020). Despite the high prevalence of prediabetes and T2DM, community-based interventions targeting lifestyle modification remain limited, with most health programs focused on treatment rather than prevention. This gap is significant, considering that early interventions can prevent or delay progression to diabetes, reduce healthcare costs, and improve quality of life (Ogurtsova et al., 2017).
Several studies in high- and middle-income countries have demonstrated the effectiveness of community-based lifestyle interventions in improving glycemic control among prediabetic individuals (Li et al., 2008; Gillies et al., 2007). These interventions often include structured exercise programs, dietary counseling, health education sessions, peer support, and regular monitoring of blood glucose. However, there is a scarcity of similar interventions in Nigeria, particularly in Ibadan communities, which limits the ability to assess the effectiveness of culturally and contextually appropriate strategies for glycemic control.
This study seeks to examine the impact of community-based lifestyle interventions on glycemic control among adults with prediabetes in Ibadan communities. By doing so, it aims to provide evidence that can guide public health policies and community health programs in Nigeria, focusing on prevention strategies that are cost-effective, culturally relevant, and scalable.
1.2 Statement of the Problem
Prediabetes affects a significant portion of the adult population in Nigeria, particularly in urban areas such as Ibadan, yet awareness, early diagnosis, and preventive interventions remain limited. Individuals with prediabetes are often asymptomatic, which results in delayed identification and inadequate management (Tabák et al., 2012). Without intervention, a large proportion of these individuals progress to type 2 diabetes mellitus, which is associated with serious complications such as cardiovascular disease, renal impairment, and neuropathy (Saeedi et al., 2019).
Despite evidence supporting lifestyle modification as a primary strategy for preventing diabetes, community-based programs in Ibadan remain sparse and fragmented. Most existing health programs focus on treatment rather than prevention, and public health messaging about diet, physical activity, and weight management is often insufficient or inconsistent (Adegoke et al., 2020). Additionally, there is limited empirical data on the effectiveness of lifestyle interventions within the local sociocultural and environmental context of Ibadan communities.
Therefore, there is an urgent need to assess the impact of community-based lifestyle interventions on glycemic control among adults with prediabetes in Ibadan. This study will provide evidence on the effectiveness of preventive strategies, identify barriers and facilitators to lifestyle modification, and inform policy and program design for reducing the progression of prediabetes to diabetes in Nigerian communities.
1.3 Research Objectives
The main objective of this study is to evaluate the impact of community-based lifestyle interventions on glycemic control among adults with prediabetes in Ibadan communities. The specific objectives are:
- To assess the baseline glycemic levels of adults with prediabetes in selected Ibadan communities.
- To implement a community-based lifestyle intervention focusing on diet, physical activity, and health education.
- To evaluate the change in glycemic control among participants after the intervention.
- To identify factors influencing the effectiveness of community-based lifestyle interventions among adults with prediabetes.
1.4 Research Questions
- What are the baseline glycemic levels of adults with prediabetes in selected Ibadan communities?
- What changes in glycemic control are observed among adults with prediabetes following community-based lifestyle interventions?
- What lifestyle and sociodemographic factors influence the effectiveness of the intervention?
- How can community-based lifestyle interventions be optimized to prevent the progression of prediabetes to diabetes in Ibadan communities?
1.5 Research Hypothesis
The study will test the following hypothesis at a 0.05 significance level:
H₀: Community-based lifestyle interventions have no significant impact on glycemic control among adults with prediabetes in Ibadan communities.
H₁: Community-based lifestyle interventions have a significant impact on glycemic control among adults with prediabetes in Ibadan communities.
1.6 Significance of the Study
This study is significant for the following reasons:
- Public Health Policy: The findings will provide evidence for policymakers and public health planners to prioritize preventive strategies for prediabetes in urban Nigerian communities.

1.7 Scope of the Study
This study will focus on adults aged 18 years and above diagnosed with prediabetes in selected communities of Ibadan, Oyo State. The study will implement a structured lifestyle intervention comprising dietary counseling, physical activity promotion, and health education over a defined period. The primary outcome measured will be glycemic control, including fasting blood glucose and HbA1c levels. The study will not include individuals with established diabetes, gestational diabetes, or severe comorbid conditions.
1.8 Definition of Terms
- Prediabetes: A condition in which blood glucose levels are higher than normal but below the threshold for diabetes diagnosis (ADA, 2023).
- Glycemic Control: The maintenance of blood glucose levels within a normal or target range through lifestyle or pharmacological interventions.
- Community-Based Intervention: A program implemented at the community level that engages participants in structured activities to improve health outcomes.
- Lifestyle Modification: Changes in diet, physical activity, and behavior aimed at improving health and preventing disease.
- Fasting Blood Glucose: Measurement of blood sugar levels after an overnight fast, used to assess glycemic status.
- HbA1c: A laboratory test that reflects average blood glucose levels over the previous 2–3 months.
- Type 2 Diabetes Mellitus (T2DM): A chronic metabolic disorder characterized by high blood glucose levels due to insulin resistance or inadequate insulin secretion (IDF, 2022).
