INTRODUCTION
1.1 Background of the Study
Malaria remains one of the most significant public health challenges globally, particularly in sub-Saharan Africa where it contributes substantially to maternal and neonatal morbidity and mortality. According to the World Health Organization (WHO, 2023), pregnant women are among the most vulnerable groups affected by malaria due to physiological and immunological changes that occur during pregnancy. These changes reduce a woman’s immunity, making her more susceptible to malaria infection and its complications.
Malaria in pregnancy is primarily caused by Plasmodium falciparum, the most virulent malaria parasite species prevalent in Nigeria. The infection often leads to sequestration of parasitized red blood cells in the placenta, a condition known as placental malaria, which interferes with nutrient and oxygen transfer to the fetus (Desai et al., 2007). One of the most common and severe consequences of malaria infection during pregnancy is maternal anaemia, which is characterized by a reduced level of haemoglobin in the blood.
Maternal anaemia is a widespread condition in developing countries and is defined by the World Health Organization as haemoglobin levels below 11 g/dL in pregnant women. Anaemia in pregnancy can result from multiple factors including nutritional deficiencies (iron, folate, vitamin B12), infections such as malaria and helminthiasis, and chronic diseases. However, malaria remains a leading cause of anaemia in endemic regions due to hemolysis (destruction of red blood cells), suppression of erythropoiesis (red blood cell production), and increased clearance of infected and uninfected erythrocytes (Menendez et al., 2000).
The severity of malaria infection is often assessed by parasite density, which refers to the number of malaria parasites per microliter of blood. Higher parasite density is typically associated with more severe clinical manifestations, including severe anaemia. Studies have shown that as parasite density increases, the degree of haemolysis and bone marrow suppression also increases, thereby exacerbating anaemia (Guyatt & Snow, 2001). Therefore, understanding the relationship between malaria parasite density and maternal anaemia is critical for improving maternal health outcomes.
In Nigeria, malaria accounts for approximately 11% of maternal deaths and significantly contributes to poor pregnancy outcomes such as low birth weight, preterm delivery, and stillbirth (National Malaria Elimination Programme [NMEP], 2022). Despite ongoing interventions such as intermittent preventive treatment in pregnancy (IPTp) with sulfadoxine-pyrimethamine, use of insecticide-treated nets (ITNs), and improved antenatal care services, malaria and its complications remain highly prevalent.
Ahmadu Bello University Teaching Hospital (ABUTH), located in Zaria, Kaduna State, serves a large population of pregnant women from both urban and rural areas. The burden of malaria and anaemia among pregnant women attending antenatal clinics in this facility reflects the broader epidemiological situation in Northern Nigeria. However, there is limited empirical data specifically examining how varying levels of malaria parasite density influence the severity of maternal anaemia in this population.
Understanding this relationship is crucial for clinicians and public health practitioners, as it can guide early diagnosis, risk stratification, and targeted interventions for pregnant women at higher risk of severe anaemia. This study therefore seeks to investigate the relationship between malaria parasite density and maternal anaemia among pregnant women attending Ahmadu Bello University Teaching Hospital.
1.2 Statement of the Problem
Malaria and maternal anaemia continue to pose serious health risks to pregnant women in Nigeria, despite the availability of preventive and therapeutic interventions. Maternal anaemia is associated with increased risks of maternal mortality, reduced physical capacity, impaired immune function, and adverse pregnancy outcomes such as low birth weight and perinatal mortality.
Although malaria has been identified as a major contributor to maternal anaemia, the extent to which parasite density influences the severity of anaemia remains inadequately explored in many healthcare settings, including Ahmadu Bello University Teaching Hospital. Most existing studies focus on the prevalence of malaria or anaemia independently, without adequately examining the quantitative relationship between parasite load and haemoglobin levels.
Furthermore, variations in parasite density among infected pregnant women may lead to differences in clinical outcomes. Some women with low parasite density may present with mild or no anaemia, while others with high parasite density may develop severe anaemia requiring urgent medical intervention. Without clear understanding of this relationship, healthcare providers may not be able to accurately predict or manage anaemia in malaria-infected pregnant women.
In addition, factors such as late antenatal booking, poor adherence to intermittent preventive treatment, and socio-economic constraints further complicate the management of malaria and anaemia in pregnancy. The lack of localized data on parasite density and its impact on maternal haemoglobin levels limits evidence-based decision-making and targeted healthcare interventions.
Therefore, this study seeks to fill this gap by examining the relationship between malaria parasite density and maternal anaemia among pregnant women attending Ahmadu Bello University Teaching Hospital. The findings will provide valuable insights for improving maternal healthcare services and reducing the burden of malaria-related complications in pregnancy.
1.3 Objectives of the Study
The general objective of this study is to examine the relationship between malaria parasite density and maternal anaemia in pregnancy.
The specific objectives are to:
- Determine the prevalence of malaria parasitaemia among pregnant women attending ABUTH.
- Assess the levels of malaria parasite density among the study population.
- Determine the prevalence of maternal anaemia among the respondents.
- Examine the relationship between malaria parasite density and haemoglobin levels.
1.4 Research Questions

H₀: There is no significant relationship between malaria parasite density and maternal anaemia among pregnant women attending Ahmadu Bello University Teaching Hospital.
H₁: There is a significant relationship between malaria parasite density and maternal anaemia among pregnant women attending Ahmadu Bello University Teaching Hospital.
1.6 Significance of the Study
This study will be beneficial to healthcare providers, policymakers, researchers, and pregnant women. It will help clinicians understand how parasite density influences anaemia severity, thereby improving diagnosis and treatment strategies.
The findings will assist policymakers in designing more effective malaria control programs targeted at pregnant women. It will also contribute to existing literature and serve as a reference for future research in maternal health and infectious diseases.
Furthermore, pregnant women will benefit from improved antenatal care services informed by evidence-based practices aimed at reducing malaria-related complications.
1.7 Scope of the Study
This study focuses on pregnant women attending antenatal clinics at Ahmadu Bello University Teaching Hospital, Zaria. It examines malaria parasite density and its relationship with maternal anaemia. Other causes of anaemia such as nutritional deficiencies and chronic illnesses will not be extensively investigated.
