Project – Effects of HIV-Related Stigma on Sleep Disorders and Visual/Neurological Health Problems among People Living with HIV in Nigeria

CHAPTER ONE

INTRODUCTION

1.1 Background of the Study

Human Immunodeficiency Virus (HIV) remains a major global public health challenge, particularly in Sub-Saharan Africa, where Nigeria bears a significant burden of infection. Despite advances in antiretroviral therapy (ART) that have transformed HIV into a manageable chronic condition, people living with HIV (PLHIV) continue to face significant psychosocial, neurological, and physiological complications that negatively affect quality of life.

One of the most persistent barriers to effective HIV management is HIV-related stigma, which refers to negative attitudes, discrimination, and social rejection directed toward individuals based on their HIV status. Stigma may be external (experienced discrimination), internalized (self-stigma), or anticipated (fear of discrimination). It affects healthcare access, treatment adherence, and psychological wellbeing. According to UNAIDS, stigma remains one of the greatest obstacles to achieving HIV epidemic control globally (UNAIDS, 2023).

In Nigeria, HIV-related stigma continues to be widespread due to cultural misconceptions, religious beliefs, and lack of accurate information. Many individuals living with HIV experience isolation, rejection, and discrimination within families, workplaces, and healthcare environments. These experiences create chronic psychological stress, which has been strongly linked to mental health disorders such as depression, anxiety, and chronic stress-related conditions.

Emerging evidence shows that HIV-related stigma does not only affect mental health but also contributes to neurological and physiological complications, including sleep disorders and neurocognitive impairment. Sleep disturbance is highly prevalent among PLHIV, characterized by insomnia, poor sleep quality, sleep fragmentation, and altered sleep architecture. Studies have shown that HIV infection itself, antiretroviral therapy, and psychosocial stressors—including stigma—contribute to disrupted sleep patterns (Osiyemi et al., 2022; WHO, 2021).

Neurologically, HIV can affect the central nervous system through chronic inflammation, viral persistence in brain reservoirs, and immune dysregulation, leading to conditions such as HIV-associated neurocognitive disorders (HAND). These neurological changes may manifest as memory impairment, reduced concentration, headaches, and visual disturbances. Furthermore, sleep disorders have been closely linked with neurocognitive decline, indicating a strong interaction between sleep health and neurological function in PLHIV (Fatokun et al., 2025).

Visual health problems are also increasingly reported among PLHIV. These include blurred vision, optic neuropathy, cytomegalovirus retinitis, and other opportunistic infections affecting the eye. Chronic stress from stigma may worsen immune suppression and increase vulnerability to such complications. Additionally, poor sleep quality has been associated with impaired visual processing and reduced neurological recovery, further worsening health outcomes in PLHIV.

The relationship between HIV-related stigma, sleep disorders, and neurological/visual complications is therefore complex and multidimensional. Stigma increases psychological stress, which disrupts sleep regulation. Poor sleep, in turn, affects immune function and neurological health, creating a cycle of physical and psychological deterioration. However, in Nigeria, limited empirical research has explored this interrelationship in detail, particularly within clinical populations.

This study therefore seeks to examine the effects of HIV-related stigma on sleep disorders and visual/neurological health problems among people living with HIV in Nigeria.

1.2 Statement of the Problem

Despite improvements in HIV treatment and care, HIV-related stigma remains highly prevalent in Nigeria, significantly affecting the wellbeing of people living with HIV/AIDS. Many PLHIV continue to experience discrimination in healthcare facilities, workplaces, and communities, which leads to emotional distress and reduced quality of life.

One major concern is that stigma contributes to chronic psychological stress, which has been strongly associated with sleep disorders. Individuals experiencing HIV stigma often report insomnia, difficulty maintaining sleep, frequent night awakenings, and poor sleep quality. Sleep disturbance has serious implications for immune function, treatment adherence, and overall health outcomes.

In addition, sleep disorders among PLHIV are closely linked with neurological complications, including cognitive impairment, memory loss, and reduced mental alertness. HIV itself affects the central nervous system, and when combined with stress induced by stigma, the risk of neurocognitive decline increases significantly. This can result in reduced functional ability and poor quality of life.

Furthermore, PLHIV are at increased risk of visual health problems, including opportunistic eye infections, blurred vision, and optic nerve damage. However, the extent to which HIV-related stigma contributes indirectly to these visual and neurological complications—through mechanisms such as stress and sleep disruption—has not been adequately studied in Nigeria.

Although several studies have examined HIV-related stigma and mental health independently, there is a lack of integrated research focusing on the combined effect of stigma on sleep disorders and neurological/visual health outcomes among PLHIV. Most existing HIV care programs in Nigeria prioritize antiretroviral treatment while neglecting sleep health and neurological assessment.

This gap in knowledge limits the development of comprehensive interventions that address both psychological and physiological complications of HIV. Therefore, this study seeks to investigate the effects of HIV-related stigma on sleep disorders and visual/neurological health problems among people living with HIV in Nigeria.

1.3 Objectives of the Study

General Objective

To examine the effects of HIV-related stigma on sleep disorders and visual/neurological health problems among people living with HIV in Nigeria.

Specific Objectives

  1. To assess the level of HIV-related stigma among PLHIV in Nigeria.
  2. To determine the prevalence of sleep disorders among PLHIV.
  3. To examine the relationship between HIV-related stigma and sleep disorders.
  4. To investigate the association between HIV-related stigma and neurological health problems among PLHIV.
  5. To assess the relationship between HIV-related stigma and visual health complications among PLHIV.

1.4 Research Questions

  1. What is the level of HIV-related stigma among PLHIV in Nigeria?
  2. What is the prevalence of sleep disorders among PLHIV?
  3. Is there a relationship between HIV-related stigma and sleep disorders?
  4. Does HIV-related stigma influence neurological health problems among PLHIV?
  5. What is the association between HIV-related stigma and visual health complications among PLHIV?

1.5 Research Hypothesis

H₀ : There is no significant relationship between HIV-related stigma and sleep disorders as well as visual/neurological health problems among people living with HIV in Nigeria.

H₁ : There is a significant relationship between HIV-related stigma and sleep disorders as well as visual/neurological health problems among people living with HIV in Nigeria.

1.6 Significance of the Study

This study is significant to several stakeholders. First, it will contribute to existing literature on the psychosocial and neurological effects of HIV-related stigma, particularly in Nigeria.

Secondly, healthcare providers will benefit from the findings by improving holistic HIV care that integrates mental health, sleep assessment, and neurological evaluation into routine HIV management.

Thirdly, policymakers and public health organizations will use the findings to strengthen stigma reduction programs and improve the quality of life of PLHIV through comprehensive care strategies.

Finally, the study will serve as a reference for future researchers in HIV/AIDS, neuropsychology, sleep medicine, and public health.

1.7 Scope of the Study

This study is limited to people living with HIV/AIDS receiving care in selected health facilities in Nigeria. It focuses specifically on HIV-related stigma and its effects on sleep disorders and visual/neurological health outcomes. Other medical complications of HIV not related to neurological or sleep health are excluded from the scope.

1.8 Operational Definition of Terms
