Project – Impact of Health Education Programmes on Maternal Healthcare Utilization Among Pregnant Women Attending Primary Healthcare Centres in Agege Local Government Area, Lagos State, Nigeria

CHAPTER ONE

INTRODUCTION

1.1 Background of the Study

Maternal healthcare remains a critical component of public health systems worldwide because the health and survival of women during pregnancy, childbirth, and the postpartum period directly influence the wellbeing of families, communities, and future generations. Pregnancy and childbirth are natural physiological processes; however, they can become life-threatening when women lack access to quality maternal healthcare services, including antenatal care, skilled birth attendance, emergency obstetric care, and postnatal services. The World Health Organization (WHO) identifies maternal mortality reduction as a major global health priority and emphasizes that most maternal deaths are preventable through timely access to appropriate healthcare services, effective health education, and improved health systems (World Health Organization [WHO], 2023).

Globally, maternal mortality has declined significantly over the past decades due to improvements in healthcare access, reproductive health interventions, and increased awareness of safe motherhood practices. Nevertheless, maternal deaths remain unacceptably high, particularly in low- and middle-income countries. According to the WHO, approximately 287,000 women died from pregnancy-related causes in 2020, with sub-Saharan Africa accounting for the highest proportion of global maternal deaths (WHO et al., 2023). The major causes of maternal mortality include severe bleeding, hypertensive disorders of pregnancy, infections, unsafe abortion, and complications during delivery. Many of these causes can be prevented or effectively managed when women utilize available maternal healthcare services.

Maternal healthcare utilization refers to the extent to which pregnant women access and use essential health services throughout pregnancy, delivery, and the postpartum period. These services include early registration for antenatal care, attendance at recommended antenatal visits, skilled delivery services, access to immunization, nutritional counselling, screening for pregnancy complications, and postnatal follow-up. Adequate utilization of maternal healthcare services improves maternal and neonatal outcomes by enabling early detection of complications, promoting healthy pregnancy behaviours, and ensuring timely medical interventions (Kruk et al., 2018).

Despite the importance of maternal healthcare utilization, many women in developing countries continue to face barriers that prevent them from accessing or fully utilizing available services. These barriers include inadequate knowledge about pregnancy care, cultural beliefs, financial constraints, poor accessibility to healthcare facilities, negative attitudes of healthcare workers, and misconceptions about antenatal and delivery services (Bohren et al., 2019). Among these factors, lack of accurate health information remains a major challenge because women who are unaware of the importance of maternal healthcare services may delay seeking care or fail to comply with recommended practices.

Health education has been identified as an effective strategy for improving maternal health outcomes by increasing women’s knowledge, changing harmful beliefs, and encouraging positive health-seeking behaviours. Health education programmes provide pregnant women with essential information regarding antenatal care attendance, danger signs during pregnancy, nutrition, birth preparedness, family planning, hygiene practices, breastfeeding, and newborn care. Through counselling sessions, community outreach, group discussions, and facility-based education, health education empowers women to make informed decisions regarding their health and that of their babies (WHO, 2016).

The effectiveness of maternal health education programmes is based on the principle that informed individuals are more likely to adopt preventive health behaviours and utilize available healthcare services. The Health Belief Model suggests that individuals’ decisions to seek healthcare are influenced by their perceptions of disease severity, susceptibility, benefits of action, and barriers to accessing care (Rosenstock et al., 1988). In maternal healthcare, women who understand the risks associated with pregnancy complications and recognize the benefits of antenatal care are more likely to attend healthcare facilities regularly and seek skilled assistance during childbirth.

In sub-Saharan Africa, improving maternal healthcare utilization remains a significant public health challenge. Although many countries have expanded primary healthcare services and introduced maternal health promotion initiatives, utilization rates remain below international recommendations. Research indicates that women’s educational status, household income, decision-making power, distance to health facilities, and exposure to health information significantly influence maternal healthcare utilization (Bobo et al., 2017). Consequently, health education interventions targeting pregnant women have become essential components of maternal health improvement strategies.

Nigeria continues to experience a high burden of maternal mortality despite various governmental and international efforts aimed at improving maternal health services. As one of the countries contributing significantly to global maternal deaths, Nigeria faces challenges related to inadequate access to quality healthcare, low utilization of skilled maternal services, socioeconomic inequalities, and insufficient health awareness among women of reproductive age. The Nigeria Demographic and Health Survey (NDHS) reported that maternal health service utilization remains uneven, with significant variations based on geographical location, education, wealth status, and urban-rural differences (National Population Commission [NPC] & ICF, 2019).

Although Lagos State has relatively better healthcare infrastructure compared with many Nigerian states due to its urban setting and economic advantages, maternal healthcare challenges still exist. The rapid population growth, overcrowding, socioeconomic disparities, and differences in health-seeking behaviours among residents create complex maternal health needs. Some women, particularly those in densely populated communities, may experience delayed antenatal registration, irregular clinic attendance, reliance on traditional birth attendants, or inadequate preparation for childbirth due to limited awareness or social barriers.

Agege Local Government Area (LGA) represents an important setting for examining maternal healthcare utilization because it is a densely populated urban area with diverse socioeconomic groups. The presence of primary healthcare centres provides opportunities for pregnant women to receive antenatal services, health counselling, preventive care, and referrals for complicated cases. However, the availability of healthcare facilities does not automatically guarantee effective utilization. Women’s knowledge, perceptions, cultural beliefs, and experiences with healthcare services significantly influence whether they attend and continue using maternal healthcare services.

Primary healthcare centres serve as the first point of contact between communities and the healthcare system. They play a vital role in maternal health promotion by providing antenatal education, routine monitoring, immunization, nutritional counselling, and reproductive health information. The success of these services depends partly on effective communication between healthcare providers and pregnant women. Health education programmes conducted at primary healthcare centres can address knowledge gaps, encourage early antenatal booking, improve compliance with healthcare recommendations, and promote safe motherhood practices.

Previous studies have demonstrated that structured health education interventions can positively influence maternal healthcare behaviours. Women who receive adequate health education are more likely to attend antenatal clinics, recognize pregnancy danger signs, prepare adequately for delivery, and utilize skilled healthcare providers. However, the effectiveness of such programmes may vary depending on implementation quality, frequency of sessions, cultural relevance, and community participation (Afulani et al., 2019).

Despite ongoing maternal health promotion efforts in Lagos State, there remains limited evidence regarding the specific impact of health education programmes on maternal healthcare utilization among pregnant women attending primary healthcare centres in Agege LGA. Understanding whether these programmes effectively influence antenatal attendance, skilled healthcare utilization, and maternal health behaviours is necessary for improving service delivery and designing targeted interventions.

Therefore, this study seeks to assess the impact of health education programmes on maternal healthcare utilization among pregnant women attending primary healthcare centres in Agege Local Government Area, Lagos State, Nigeria. The findings will provide valuable information on the effectiveness of maternal health education initiatives and contribute to strategies aimed at improving maternal and child health outcomes.

1.2 Statement of the Problem

Maternal mortality remains one of the major public health concerns in Nigeria despite decades of interventions aimed at improving maternal healthcare services. Although many maternal deaths are preventable through timely utilization of antenatal care, skilled delivery services, and emergency obstetric interventions, a considerable number of Nigerian women continue to experience pregnancy-related complications due to delayed healthcare seeking and inadequate use of available maternal health services. This situation indicates that increasing the availability of healthcare facilities alone may not be sufficient without addressing behavioural and informational factors influencing healthcare utilization.

One of the major challenges affecting maternal healthcare utilization is inadequate knowledge among pregnant women regarding the importance of antenatal care, birth preparedness, pregnancy danger signs, and skilled attendance during delivery. Some women begin antenatal care late, attend irregularly, or fail to seek professional assistance during childbirth because they lack sufficient information about potential pregnancy risks. Inadequate knowledge may also encourage reliance on untrained birth attendants or traditional practices that increase maternal and neonatal risks.

Although health education programmes have been introduced in many primary healthcare facilities to improve maternal awareness and encourage positive health behaviours, questions remain regarding their actual effectiveness in changing healthcare utilization patterns. In some communities, women continue to demonstrate poor attendance at antenatal clinics and low uptake of skilled maternal services despite the availability of health education sessions. This suggests that factors beyond programme availability, such as quality of communication, cultural beliefs, socioeconomic conditions, and healthcare experiences, may influence outcomes.

Agege Local Government Area faces unique maternal health challenges associated with high population density, socioeconomic diversity, and pressure on available healthcare resources. While primary healthcare centres provide maternal health education and related services, there is insufficient empirical evidence on whether these programmes have significantly improved healthcare utilization among pregnant women in the area. Without such evidence, healthcare planners may find it difficult to determine whether existing health education strategies are effective or require modification.

Furthermore, previous studies on maternal healthcare utilization in Nigeria have largely focused on national patterns, rural communities, or general determinants of antenatal care attendance. Limited attention has been given to assessing the direct influence of health education programmes among pregnant women accessing primary healthcare services within specific urban local government areas such as Agege. This creates a contextual gap that needs to be addressed.

Therefore, this study is necessary to examine the impact of health education programmes on maternal healthcare utilization among pregnant women attending primary healthcare centres in Agege Local Government Area, Lagos State. The study will provide evidence on whether health education initiatives contribute to improved maternal healthcare behaviours and identify areas requiring further improvement.
