INTRODUCTION
1.1 Background of the Study
Diabetes mellitus is a chronic metabolic disorder characterized by hyperglycemia resulting from defects in insulin secretion, insulin action, or both (American Diabetes Association [ADA], 2023). The disease is associated with long-term damage, dysfunction, and failure of various organs, particularly the eyes, kidneys, nerves, heart, and blood vessels. Type 2 diabetes mellitus (T2DM) is the most common form of diabetes, accounting for over 90% of cases worldwide, and is strongly associated with obesity, sedentary lifestyles, unhealthy dietary habits, and aging populations (International Diabetes Federation [IDF], 2022).
Globally, diabetes is a significant public health challenge. The IDF (2022) estimates that 537 million adults live with diabetes, with a projected increase to 783 million by 2045. A considerable proportion of these cases are in low- and middle-income countries, where healthcare systems are often ill-equipped to manage chronic diseases (Saeedi et al., 2019). In Nigeria, diabetes has emerged as a major health concern. The prevalence of T2DM among Nigerian adults is estimated to be between 5% and 10%, with regional variations and higher rates in urban areas due to lifestyle changes associated with urbanization (Uloko et al., 2012).
Urbanization in Nigeria has brought about significant shifts in dietary patterns, reduced physical activity, increased stress levels, and higher prevalence of obesity, all of which contribute to the rising incidence of T2DM (Kadiri et al., 2018). Conversely, rural communities face different challenges, including limited access to healthcare facilities, lack of health education, and socio-economic barriers that may hinder early diagnosis and proper management of diabetes (Uloko et al., 2012). These factors, coupled with genetic predispositions, underscore the complex interplay of lifestyle, environmental, and hereditary determinants of T2DM in Nigeria.
The burden of T2DM extends beyond individual health, affecting families, communities, and the healthcare system. Individuals with poorly managed diabetes are at increased risk of complications such as cardiovascular disease, neuropathy, nephropathy, and retinopathy, which not only reduce quality of life but also increase healthcare costs and mortality rates (Ekoru et al., 2019). Furthermore, the social and economic consequences of diabetes in Nigeria include loss of productivity, increased medical expenditures, and reduced workforce participation, making it a pressing public health issue.
Despite these challenges, there is limited recent research that directly compares the prevalence and risk factors of T2DM in urban versus rural communities in Nigeria. Such data are critical to designing context-specific interventions that consider the unique socio-cultural and economic realities of these populations. Understanding the differences in risk factor profiles between urban and rural settings can inform targeted health education programs, community-based interventions, and policy formulation aimed at preventing and managing T2DM effectively (Kadiri et al., 2018).
1.2 Statement of the Problem
Type 2 diabetes mellitus is increasingly recognized as a significant health problem in Nigeria, with both urban and rural populations experiencing a rising burden. Urban residents often adopt lifestyles characterized by physical inactivity, unhealthy diets, high consumption of processed foods, and increased stress levels, which are established risk factors for T2DM (IDF, 2022). On the other hand, rural populations face limited health education, poor awareness of the disease, restricted access to medical care, and cultural practices that may delay diagnosis and management (Uloko et al., 2012).
Despite the growing prevalence of T2DM in Nigeria, current data remain fragmented, with few studies providing a direct comparison between urban and rural communities. This gap hinders effective public health planning and the development of targeted interventions. Without comprehensive data, healthcare providers may fail to address the specific needs of different populations, leading to suboptimal prevention and treatment outcomes.
Furthermore, the lack of information on socio-demographic and lifestyle risk factors across diverse communities limits the ability of policymakers and healthcare practitioners to design contextually relevant strategies. There is a need to identify how factors such as age, gender, body mass index (BMI), physical activity, dietary patterns, and family history influence the prevalence of T2DM in both urban and rural settings. This study seeks to address these gaps by investigating the prevalence and risk factors of T2DM among adults in selected urban and rural communities in Nigeria, providing evidence to support effective public health interventions.
1.3 Research Objectives
The main objective of this study is to assess the prevalence and risk factors of type 2 diabetes mellitus among adults in urban and rural communities in Nigeria. The specific objectives are:
- To determine the prevalence of type 2 diabetes mellitus among adults in selected urban and rural communities in Nigeria.
- To identify the socio-demographic and lifestyle risk factors associated with T2DM among adults in these communities.
- To compare the prevalence and risk factors of T2DM between urban and rural populations.
- To provide recommendations for targeted interventions and public health strategies for T2DM prevention and management.
1.4 Research Questions
- What is the prevalence of type 2 diabetes mellitus among adults in selected urban and rural communities in Nigeria?

1.5 Research Hypothesis
The study will test the following hypothesis at a 0.05 significance level:
H₀: There is no significant difference in the prevalence and risk factors of type 2 diabetes mellitus between adults in urban and rural communities in Nigeria.
H₁: There is a significant difference in the prevalence and risk factors of type 2 diabetes mellitus between adults in urban and rural communities in Nigeria.
1.6 Significance of the Study
The findings of this study will be valuable in several ways:

1.7 Scope of the Study
This study will focus on adults aged 18 years and above in selected urban and rural communities in Nigeria. The study will examine the prevalence of T2DM and associated socio-demographic and lifestyle risk factors, such as age, gender, BMI, physical activity, diet, and family history of diabetes. The study will not cover type 1 diabetes, gestational diabetes, or populations outside the selected communities.
1.8 Definition of Terms
- Diabetes Mellitus: A chronic metabolic disorder characterized by high blood glucose levels due to defects in insulin secretion or action (ADA, 2023).
- Type 2 Diabetes Mellitus (T2DM): A form of diabetes usually occurring in adults, often associated with obesity, physical inactivity, and insulin resistance (IDF, 2022).
