INTRODUCTION

1.1 Background of the Study

Maternal and child health (MCH) remains a pressing public health concern in sub-Saharan Africa (SSA), a region that continues to bear the brunt of the global burden of maternal and under-five mortality. According to the World Health Organization (WHO, 2023), SSA accounts for approximately 66% of maternal deaths worldwide, with most of these deaths being preventable through timely and effective healthcare interventions. Similarly, child mortality rates remain high, driven by complications during childbirth, infectious diseases, and poor postnatal care. Despite global initiatives such as the Millennium Development Goals (MDGs) and their successor, the Sustainable Development Goals (SDGs), SSA has struggled to make uniform progress in reducing maternal and child mortality across countries due to disparities in infrastructure, governance, and healthcare financing.

Several systemic barriers contribute to the stagnation in MCH outcomes across SSA. These include limited access to quality healthcare services, particularly in rural and underserved regions, a critical shortage of trained health professionals, inadequate healthcare infrastructure, and persistent socio-cultural norms that discourage facility-based childbirth and child immunization (UNICEF, 2022). Additionally, logistical and financial constraints hinder the delivery of essential services such as antenatal care, skilled birth attendance, and immunization campaigns. These structural weaknesses compromise the capacity of health systems to respond to the needs of mothers and children, resulting in avoidable deaths and long-term health complications.

In the past two decades, digital technologies have emerged as innovative solutions to many of these challenges. Digital health interventions encompass a wide range of tools, including mobile health (mHealth) applications, SMS-based reminders, telemedicine, electronic health records (EHRs), health information systems, and artificial intelligence (AI) applications. These technologies aim to bridge the gaps in healthcare access and delivery by supporting remote consultations, streamlining data collection and reporting, enhancing communication between patients and healthcare providers, and improving the efficiency of service delivery. The ability of these tools to reach even the most remote populations positions them as powerful mechanisms for addressing longstanding MCH disparities.

The rapid expansion of mobile phone ownership and internet connectivity across SSA has significantly enhanced the feasibility and reach of digital health interventions. According to GSMA (2021), mobile phone penetration in SSA surpassed 50% by 2020, with mobile internet usage steadily increasing even in rural communities. This digital transformation has enabled governments, NGOs, and healthcare institutions to deploy mobile-based health initiatives at scale. For example, SMS reminder systems for antenatal visits, mobile apps for monitoring pregnancy complications, and teleconsultation platforms have been introduced in countries like Kenya, Nigeria, and Rwanda. These interventions are often cost-effective and user-friendly, making them particularly suitable for low-resource settings.

Between 2004 and 2024, a substantial body of research has emerged evaluating the impact of digital interventions on various maternal and child health outcomes in SSA. Studies have reported improvements in indicators such as antenatal care attendance, skilled birth attendance, vaccination coverage, and maternal knowledge of health practices. However, the results are often heterogeneous. Many digital health programs remain pilot or donor-driven, with limited integration into national health systems and variable success in achieving long-term sustainability. Additionally, questions remain about the contextual factors—such as digital literacy, cultural acceptability, and political will—that influence the effectiveness of these interventions.

Given the growing importance of digital health in shaping the future of maternal and child healthcare in SSA, there is an urgent need for comprehensive evidence synthesis. A systematic review and meta-analysis that consolidates findings from the past two decades will provide critical insights into which digital interventions work, under what conditions, and for whom. Such an evidence base is essential to inform policymakers, health system planners, donors, and other stakeholders aiming to scale up effective, equitable, and sustainable digital health solutions for maternal and child health improvement in SSA.

1.2 Statement of the Problem

Despite the increasing adoption of digital innovations to improve maternal and child health (MCH) outcomes in sub-Saharan Africa (SSA), the region continues to face disproportionately high rates of maternal and under-five mortality. According to the World Health Organization (2023), maternal mortality stands at an alarming 545 deaths per 100,000 live births, while under-five mortality is estimated at 72 deaths per 1,000 live births. These statistics underscore a persistent health crisis, suggesting that digital interventions—though promising—have not yet achieved widespread or consistent impact across the region.

While digital health tools such as mobile health applications, SMS reminders, electronic health records, and telemedicine platforms have been introduced to address critical gaps in service delivery, several systemic barriers limit their effectiveness. Key among these are unequal access to digital technologies, especially in rural or impoverished communities; low levels of digital and health literacy among users; inadequate technological infrastructure such as poor internet connectivity; and a lack of trained personnel to manage and utilize digital systems effectively (Ojo et al., 2020). Furthermore, many digital health initiatives are implemented as isolated pilot projects, lacking integration into national healthcare systems and often ceasing once donor funding ends.

Another major issue is the fragmentation of evidence concerning the effectiveness of digital interventions in improving MCH outcomes in SSA. Although numerous studies have been conducted, they vary in scope, quality, and methodological rigor, making it difficult to draw clear conclusions about what works, where, and why. The absence of a consolidated knowledge base poses a significant challenge for policy-makers, health practitioners, and development partners seeking to scale up successful interventions or allocate resources efficiently.

Moreover, contextual factors such as cultural norms, gender dynamics, economic disparities, and political instability often influence the uptake and sustainability of digital interventions in ways that are not well understood. A lack of longitudinal data and comparative analysis across different countries and settings further obscures the long-term effectiveness and adaptability of these technologies. As a result, decision-makers are often left without reliable guidance on how to design, implement, and evaluate digital health programs that are both context-sensitive and scalable.

Given these challenges, there is a pressing need for a systematic review and meta-analysis of digital health interventions targeting maternal and child health in SSA over the past two decades. Such a study will not only synthesize existing evidence but also identify trends, best practices, success factors, and implementation barriers. The findings will offer critical insights for improving future program design, strengthening health systems, and ultimately reducing preventable maternal and child deaths in the region.

1.3 Objectives of the Study

The main objective of this study is to systematically review and analyze the effectiveness of digital interventions in improving maternal and child health outcomes in sub-Saharan Africa from 2004 to 2024.

The specific objectives are to:

1. Identify and classify digital health interventions targeted at maternal and child health in SSA.
2. Evaluate the effectiveness of these interventions in improving health outcomes such as antenatal care attendance, skilled birth attendance, immunization, and postnatal care.
3. Determine the factors influencing the success or failure of digital health interventions in SSA.
4. Synthesize findings to inform policy, practice, and future research in digital health and MCH.

1.4 Research Questions

The study is guided by the following research questions:

1. What types of digital interventions have been implemented to improve maternal and child health in SSA between 2004 and 2024?
2. How effective have these interventions been in achieving improved MCH outcomes?
3. What contextual factors facilitate or hinder the success of digital health interventions?
4. What lessons can be learned to scale up successful interventions and improve policy and practice?

1.5 Significance of the Study

The importance of this study lies in its potential to offer a comprehensive synthesis of digital health interventions targeting maternal and child health (MCH) in sub-Saharan Africa (SSA) over a 20-year period. Despite the growing interest in digital solutions within the healthcare sector, existing research is often fragmented, context-specific, or limited in scale. By conducting a systematic review and meta-analysis, this study will consolidate findings from diverse sources, offering a robust evidence base that reflects regional trends, challenges, and outcomes. This synthesis is particularly vital as SSA remains one of the regions with the highest burden of maternal and child mortality.

For policymakers, this study provides actionable insights into which digital interventions have been most effective in improving MCH outcomes and why. In a context where resources are often limited, such evidence is critical to inform strategic investments and prioritize interventions that have demonstrated cost-effectiveness and scalability. Governments and development partners can use the findings to design national policies and frameworks that integrate digital health more effectively into existing health systems, especially in under-resourced and rural areas.

Health practitioners and program implementers also stand to benefit from the findings of this research. By identifying the features of successful digital health initiatives—such as user engagement strategies, implementation modalities, and technological configurations—this study will provide a blueprint for practitioners seeking to replicate or scale up digital interventions in their own settings. Understanding what works in real-world conditions, and why, can reduce trial-and-error approaches and support the delivery of more efficient, patient-centered care.

Non-governmental organizations (NGOs) and international development agencies involved in maternal and child health programs will find the study equally useful. Many such organizations fund or implement digital health programs without clear evidence of long-term impact or sustainability. This study can help bridge that gap by highlighting best practices, flagging common pitfalls, and offering recommendations grounded in evidence. It can also aid in monitoring and evaluation processes, enabling NGOs to refine their strategies and enhance program outcomes.

Academically, the study makes a significant contribution to global health literature by addressing a long-standing knowledge gap. While there have been multiple isolated studies and small-scale reviews, few have combined both qualitative and quantitative data across two decades to examine digital interventions in the MCH context of SSA. This meta-analytic approach ensures that not only are statistical outcomes evaluated, but also that the social, economic, and health system contexts influencing those outcomes are explored. As such, the study contributes to theory-building and can serve as a foundational reference for future research.

Lastly, the study promotes evidence-based decision-making in the design, funding, and implementation of digital health interventions. In a region where health systems are often strained, and public health planning must be strategic, the availability of consolidated and contextualized evidence is essential. By translating data into knowledge and knowledge into practice, this study can help advance maternal and child health outcomes in sub-Saharan Africa and support the attainment of global health targets such as the Sustainable Development Goals.

1.6 Scope of the Study

The study focuses exclusively on digital interventions designed to improve maternal and child health outcomes within the geographical region of sub-Saharan Africa, covering the time period from January 2004 to March 2024. It includes peer-reviewed journal articles, grey literature, program evaluations, and policy documents. The scope encompasses interventions using mobile health applications, SMS reminders, decision support tools, electronic health records, and teleconsultations.

1.7 Definition of Terms

Digital Health Interventions: Digital health interventions refer to the application of digital technologies to support and improve the delivery, quality, and efficiency of healthcare services. These include a wide array of tools such as mobile health (mHealth) applications, short message service (SMS) reminders, electronic medical records (EMRs), telemedicine platforms, wearable health devices, and decision-support systems powered by artificial intelligence. In the context of maternal and child health, these technologies are used to facilitate remote consultations, provide health education, remind patients of appointments and vaccinations, collect real-time health data, and support healthcare workers in making timely clinical decisions. Digital interventions aim to overcome barriers such as geographic inaccessibility, shortage of health personnel, and poor health infrastructure—especially in low-resource settings like sub-Saharan Africa.

Maternal and Child Health (MCH): Maternal and child health refers to the overall health and well-being of women during pregnancy, childbirth, and the postpartum period, as well as the health of infants and children under five years of age. MCH is a critical component of public health as it determines the future health and development of populations. Key indicators of MCH include maternal mortality ratio, neonatal and under-five mortality rates, antenatal and postnatal care coverage, skilled birth attendance, and immunization coverage. Addressing MCH involves ensuring access to quality healthcare services, skilled healthcare providers, essential medicines, nutrition, and safe delivery practices. In sub-Saharan Africa, MCH outcomes are often undermined by poverty, poor access to healthcare, and weak health systems—challenges that digital health solutions seek to mitigate.

Systematic Review: A systematic review is a rigorous research methodology used to comprehensively collect, evaluate, and synthesize all available evidence on a specific research question or topic. Unlike traditional literature reviews, systematic reviews follow a transparent, replicable process that involves a clearly defined search strategy, inclusion and exclusion criteria, and quality assessment of the included studies. The goal is to minimize bias and provide a reliable summary of the available evidence. In this study, the systematic review approach is used to evaluate the breadth and effectiveness of digital health interventions for maternal and child health in sub-Saharan Africa over a 20-year period (2004–2024), thereby offering policymakers and practitioners a well-founded basis for decision-making.

Meta-analysis: Meta-analysis is a statistical method used in conjunction with a systematic review to quantitatively synthesize the results of multiple individual studies. It involves calculating a pooled estimate of the effect size of an intervention, which provides a more precise measurement than results from single studies. Meta-analyses can identify trends, strengthen the overall evidence base, and increase the generalizability of findings. In the context of this research, meta-analysis is employed to assess the impact of digital health interventions on maternal and child health outcomes, such as antenatal care attendance, institutional delivery, immunization rates, and child survival. The findings from the meta-analysis will help establish the overall effectiveness of these interventions and inform evidence-based health programming in the region.
